Применение сетчатых полипропиленовых имплантатов в лечении послеоперационных вентральных грыж: экспериментальное и клиническое исследование

Abstract

В эксперименте на 60 белых нелинейных крысах изучены морфологические изменения в зоне имплантации полипропиленовой сетки зарубежного производства «Surgipro Mesh» и отечественного «Эсфил» (ООО «Линтекс»). Оба материала проявили себя достаточно однородно, раны заросли первичным натяжением, совпали и сроки заживления. В дальнейшем вокруг филаментов сетки сформировалась соединительнотканная капсула, имплантат прорастал фиброзной тканью. Во 2-м хирургическом отделении Приморской краевой клинической больницы № 1 с 2003 по 2009 г. по поводу ПВГ прооперирован 151 пациент с применением сетчатых имплантатов. Дефект брюшной стенки закрывали двумя методами: в первую группу вошли 113 пациентов, которым за период с 2003 по 2007 г. выполняли аллопластику Onlay-способом с использованием полипропиленового имплантата «Surgipro Mesh». Вторую группу составили 38 больных, которым за период с 2007 по 2009 г. применялась аллопластика Onlay-способом с использованием полипропиленовой сетки «Эсфил». В ближайшем послеоперационном периоде осложнения носили общехирургический характер и в обоих случаях составили по 13%.

We have investigated morphological changes in zones of implantation of polypropylene foreign mesh (SURGIPRO Mesh) and domestic mesh «Esfil» (OOO «Lintex»). The experiments were carried out on 60 non-linear white rats. Both materials displayed similar properties, the wounds was healed by primary intention and healing duration by primary intention was the same. In the sequel connective tissue capsule formed around mesh filaments and implant germinated by fibrous tissues. There were 151 patients operated for PVH (post operational ventral hernia) using mesh implants in 2-nd surgery deparment of the Primorye territorial clinical hospital № 1 in the period from 2003 to 2009. The defects of abdominal walls were closed by two ways. The first group of 113 patients underwent alloplasty using polypropylene implant «Surgipro Mesh» by Onlay-method in the period from 2003 to 2007. The second group included 38 patients receiving alloplasty polypropylene meth «Esfil» by Onlay-methods. In the nearest postoperative period complications were of general surgical character and average 13%.
