Abstract

The article analyzes the main factors influencing the choice of the volume and the qualitative composition of the liquid for volemic resuscitation in children with severe sepsis and septic shock. We have considered the physiology of water balance and excretory system in children of different age groups, analyzed the criteria of determining the sufficient level of fluid therapy for critical hypovolemia in resuscitation phases, optimization, stabilization and evacuation of fluid. Interstitial overload by fluid and critical hypoalbuminemia adversely affect the final outcome of medical care to children with severe sepsis and septic shock.

В статті проаналізовано основні фактори, що впливають на вибір об’єму та якісного складу рідини для волемічної ресусцитації у дітей з тяжким сепсисом і септичним шоком. Розглянуто фізіологію водного балансу та системи виділення у дітей різних вікових груп, проаналізовано критерії визначення достатнього рівня інфузійної терапії при критичній гіповолемії в фазах ресусцитації, оптимізації, стабілізації й евакуації рідини. Перевантаження інтерстицію рідиною та критична гіпоальбумінемія несприятливо впливають на кінцевий результат надання медичної допомоги дітям із тяжким сепсисом і септичним шоком.
