Сравнительный анализ хирургического лечения витреофовеолярного тракционного синдрома

Abstract

Данная статья посвящена сравнительному анализу хирургического лечения витреофовеолярного тракционного синдрома по предложенной авторами методике с удалением внутренней пограничной мембраны (ВПМ) и без её удаления. Пациенты случайным образом были разделены на 2 группы. В группе 1 выполнили хирургическое лечение по предложенной методике без удаления ВПМ. В группе 2 выполняли хирургическое лечение по предложенной методике с перифовеолярным удалением ВПМ. При сравнительном анализе выявлено, что в обеих группах был достигнут анатомический результат, однако в группе 1 в 35 % случаев выявлено развитие эпиретинального фиброза в позднем послеоперационном периоде, чего не наблюдалось в группе 2. Восстановление зрительных функций в группе 1 проходило быстрее, но к сроку наблюдения в 1 год данные групп 1 и 2 были сопоставимы. Из этого следует, что применение методики хирургического лечения с удалением ВПМ более предпочтительно, в связи с более стабильным анатомическим результатом.

This article focuses on the surgical treatment of the vitreomacular traction ( VMT) syndrome comparing vitrectomy with and without internal limiting membrane (ILM) removal. Patients were randomly divided into 2 groups. In the 1st group, the procedure was performed without the ILM removal. In the 2nd group, the surgery was performed with perifoveolar ILM removal. A comparative analysis revealed that an anatomical result was reached in both groups, but in the 1st group, the development of epiretinal fibrosis was observed in 35 % of the cases in the late postoperative period; and this was not observed in the 2nd group. Visual function recovery was faster in the 1st group, but at 1 year visit, data from both groups were comparable. Thus, the treatment method with surgical ILM removal is preferable, due to more stable anatomical result.
