Антиаритмическая терапия пароксизмальных тахикардий и экстрасистолий у больных с дисфункцией синусового узла

Abstract

Лечение антиаритмическими препаратами при нарушениях автоматизма синусового узла и замедлениях атриовентрикулярного проведения может осложняться гемодинамически значимыми брадиаритмиями; вместе с тем имплантация электрокардиостимулятора не всегда возможна. Цель исследования — оценить влияние антиаритмической терапии аллапинином на функцию синусового и атриовентрикулярного узлов. Обследованы 20 больных с различными нарушениями ритма сердца на фоне дисфункции синусового узла (средний возраст 41 ± 2,3 года). Пациентам с антиаритмической целью назначали аллапинин per os в дозах 37,5—50 мг/сут. Показано, что включение в терапию аллапинина привело к выраженному антиаритмическому эффекту, что проявлялось в отношении как наджелудочковой, так и желудочковой эктопической активности, причем негативного влияния на функцию синусового и атриовентрикулярного узлов не зарегистрировано.

Antiarrhythmic therapy of patients with disturbed automatism of the sinus node and impaired atrioventricular conductance may be complicated by hemodynamically significant bradycardias and contraindications for implantation ofa cardiac electrical stimulator. This study aimed at estimating effect of antiarrhythmic therapy with allapinin on the function of sinus and atrioventricular nodes. It included 20 patients (mean age 37.5±2.3 years) with disturbed cardiac rhythm and sinus node dysfunction treated with allapinin (37.5 — 50 mg/d per os). This therapy had well apparent antiarrhythmic effect manifest as improvement of supraventricular and ventricular ectopic activities in the absence of negative influence on the function of sinus and atrioventricular nodes.
