Совершенствование пренатальной диагностики на региональном уровне: начало пути

Abstract

Objective: Studying of efficiency of the program of the combined screening of first-trimester on the basis of a district office of part of the city over the Nizhny Novgorod. Methods. From 01.01.2013 for 01.09.2014 in the District office of prenatal diagnostics of part over the river of the city of Nizhny Novgorod on the basis of female consultation No. 5 7087 women in the first trimester were examined. In 2013 at 33 markers of chromosomal abnormalities and at 23 congenital defects, in 2014 at 23 and 7 cases respectively are revealed. 9 fetuses with chromosomal abnormalities were revealed. Transition to the new program of calculation of individual risk of Astraia allowed to reduce twice the group of risk founded on the isolated biochemical markers. Measurement of the 4th ventricle of a fetal spina bifida allowed already and to suspect inclusion in the protocol of research the first trimester. Conclusions. Inclusion in an assessment of risk of additional ultrasonic markers with obtaining certificates of FMF on a ductus venous, measurement of nasal bones, a tricuspidal regurgitation, and also the organizations of feedback, for tracking of results of the work will be further improvement of work.

Цель исследования: изучение эффективности программы комбинированного скрининга 1 триместра на базе окружного кабинета заречной части города Нижний Новгород. Методы. За период с 01.01.2013 по 01.09.2014 г. в Окружном кабинете пренатальной диагностики заречной части города Нижнего Новгорода на базе ГБУЗ НО женская консультация № 5 обследовано 7087 женщин в первом триместре. В 2013 г. у 33 плодов выявлены маркеры ХА и у 23 ВПР, в 2014 г. у 23 и 7 плодов соответственно. По результатам кариотипирования ХА выявлены у 9 плодов. Переход на новую программу расчета индивидуального риска Astraia позволил уменьшить в два раза группу риска, основанную на изолированных биохимических маркерах. Включение в протокол исследования измерение 4-го желудочка плода позволил уже а первом триместре заподозрить spina bifida. Выводы. Дальнейшим совершенствованием работы ОКПД будет включение в оценку риска дополнительных ультразвуковых маркеров с получением сертификатов FMF по венозному протоку, измерению костей носа, трикуспидальной регургитации, а так же организации обратной связи, для отслеживания результатов своей работы.
