Abstract

Цель исследования: определение влияния жировой нагрузки на качество изображения при проведении сцинтиграфии миокарда в покое и с нагрузкой. Материалы и методы: проведен анализ сцинтиграмм сердца у 48 пациентов, разделенных на 4 группы. На нативных сцинтиграммах выбирались зоны интереса с областей сердца, печени, желчного пузыря, кишечника и фонового накопления. Рассчитывалось значение накопления препарата в выбранных зонах по отношению к накоплению в миокарде. Результаты: при количественной обработке данных наибольшее накопление наблюдалось в области печени, интенсивность которого после жировой нагрузки уменьшалась, но, несмотря на визуальное улучшение качества изображения, реальное соотношение между интенсивностью накопления в сердце и в печени не увеличивалось, что подтверждается отсутствием статистических различий значений между выборками. Выводы 1. При исследовании влияния жировой нагрузки на качество получаемых сцинтиграмм при проведении ОФЭКТ сердца увеличения накопления РФП в зоне миокарда выявлено не было. Получить приемлемое качество изображений, сравнимое с контрольной группой, было возможно с помощью стандартных программ обработки с использованием алгоритмов отсечки фона. 2. Пищевая нагрузка влияет только на субъективное восприятие сцинтиграмм. Отсутствие количественных различий в соотношении между накоплением РФП в сердце и в печени дает возможность отказаться от жировой нагрузки без потери данных при программной обработке изображений.

Aims and objectives: to determine fat influence on image quality during myocardial SPECT by qualitative and quantitative analysis of radiotracer accumulation in myocardium in comparison with other tissues. Materials and methods: we have analyzed heart scintigrams of 48 patients. We have selected the ROIs on scintigrams above heart, liver, intestines and background accumulation. We have calculated ratio of the agent accumulation in the selected areas to myocardium. Results: quantitative data processing showed the greatest accumulation in the liver, with decreasing intensity after FL, but, in spite of image improvement, actual ratio of accumulation intensity in the heart and in the liver did not increase, which was confirmed by lack of statistic differences between the groups of patients. Conclusions Research of FL influence on quality of scintigrams during heart SPECT analysis showed no increase of radiotracer accumulation in myocardium. Acceptable image quality comparable with that of reference group was reached by standard programs of processing with use of background cut-off algorithm. FL influences only subjective perception of scintigrams. Lack of quantitative differences in heart-to-liver ratio of the radiotracer accumulation allows refusal to use FL without data loss in image processing.
