Abstract

Background. Estimation of electrophysiological indices of asymptomatic paroxysms in pregnant women allows to choose an adequate therapy management, to rationally plan delivery. The aim of the study is examine electrophysiological indices of the conducting system of heart in pregnant and nonpregnant women with asymptomatic paroxysms of supraventricular tachycardia. Materials and methods. The authors examined the conducting system of heart in 56 patients with asymptomatic paroxysms using an “Astrocard” electrophysiological complex (CJSC “Medi-tech”). Results. It has been revealed that adaptation of the cardiovascular system to pregnancy creates conditions activating wandering focuses and retrograde circulation of impulses. It was proved that an electrophysiological substratum of retrograde circulation of activation appears to be the latent WPW syndrome, the longitudinal displacement of atrioventricular junction (AB) on α and β paths, the combination of the longitudinal displacement of atrioventricular junction (AB) on α and β paths with the latent WPW syndrome. It has been established that in nonpregnant women it is possible to induce paroxysm of supraventricular tachycardia up to 3 complexes in 53,8 % of cases, up to 5 in 7,7 %, not brought on in 38,5 %, in pregnant women in 13,3, 56,7, 30,0 % of cases, respectively. Conclusions. Pregnant and nonpregnant women with paroxysms of supraventricular tachycardia at the same incidence suffer from the latent WPW syndrome and the longitudinal displacement of atrioventricular junction (AB) on α and β paths. The beginning of pregnancy is accompanied with extension of supraventricular tachycardia duration.

Актуальность и цель. Оценка электрофизиологических показателей бессимптомных пароксизмов у беременных позволяет выбрать адекватную тактику терапии, разумно планировать родоразрешение. Целью данного исследования стало изучение электрофизиологических показателей проводящей системы сердца у небеременных и беременных женщин с бессимптомными пароксизмами суправентрикулярной тахикардии. Материал и методы. У 56 пациенток с бессимптомными пароксизмами с помощью электрофизиологического комплекса «Аstrосаrd» (ЗАО «Меди-тек») изучалась проводящая система сердца. Результаты. Выявлено, что адаптация сердечно-сосудистой системы к беременности создает условия для активации эктопических очагов возбуждения и ретроградной циркуляции импульсов. Доказано, что электрофизиологическим субстратом ретроградной циркуляции возбуждения является скрытый синдром WРW, продольная диссоциация атриовентрикулярного (АВ) соединения на α и β пути, сочетание продольной диссоциации АВ соединения на α и β пути со скрытым синдромом WРW. Установлено, что у небеременных женщин удается индуцировать пароксизм суправентрикулярной тахикардии до трех комплексов в 53,8 % случаев, до шести в 7,7 %, не вызывается в 38,5 %, у беременных в 13,3, 56,7, 30,0 % случаев соответственно. Выводы. У небеременных и беременных женщин с пароксизмами суправентрикулярной тахикардии с одинаковой частотой встречается скрытый синдром WРW и продольная диссоциация АВ соединения на α и β пути. Наступление беременности сопровождается удлинением продолжительности суправентрикулярных тахикардий.
