Abstract

В статье представлены результаты сравнительного анализа методов ЭЛ и ЭЛ+ЭС в лечении пациентов с варикозным расширением вен пищевода с целью профилактики варикозных кровотечений. Девяносто два пациента с циррозом печени рандомизированы на 2 группы: группа I (лигирования) 28 пациентов и группа II (лигирование + склеротерапия). Оба метода одинаково и высоко эффективны в достижении эрадикации вен (85,7 % против 84,4 %) и по показателю выживаемости в течение года (89,3 % против 85,9 %). Комбинированная терапия сопровождалась большим количеством лечебных сеансов до эффекта эрадикации (5,3 ±0,7 против 1,5±0,5, рВ тоже время, в течение года в группе пациентов после лигирования достоверно чаще развивались рецидив варикозных вен (50 % против 25 %, ри варикозные кровотечения (28,6 % против 14,1 %, р Оба метода могут успешно применяться для профилактики рецидива варикозного кровотечения. The aim of this study was to assess the efficacy of band ligation alone versus band ligation plus sclerotherapy in the prevention of recunent variceal bleeding in cinhotic patients. Methods: ninety two cinhotic patients were randomized to group I (loop ligation) with 28 patients or to group II (loop ligation plus sclerotherapy) with 64 patients. Results: no statistical differences were observed between group I and group II in indices of one year survival (89,3 % vs 85,9 %) and variceal eradication (85,7 % vs 84,4 %). The number of patients with recurrent varices and variceal bleeding after procedures was higher in group I than in group II (50 % vs 25 % and 28,6 % vs 14,13 %, respectively; p0,05). Conclusions: Ligation and sclerotherapy reduces variceal recurrent rate. Combined therapy is superior to ligation alone in prevention of recunent variceal bleeding.
