Изменение размеров пролактин-секретирующих макроаденом гипофиза на фоне терапии агонистами дофамина

Abstract

Пролактинсекретирующие аденомы гипофиза составляют 40% от всех макроаденом гипофиза. Медикаментозная терапия агонистами дофамина эффективна в лечении большинства больных с пролактиномой. В исследовании проведен анализ эффективности этой терапии в зависимости от возраста и пола больных, размеров и локализации опухоли, уровня пролактина. В исследование включено 176 больных (109 мужчин и 67 женщин в среднем возрасте 34 г). Длительность наблюдения и лечения составили от 3 месяцев до 3 лет (медиана 6 месяцев). У 52% больных размеры опухоли были более 36 мм, а уровень пролактина варьировал в диапазоне от 2333 мЕд/л до 2218283 мЕд/л (медиана 37735). Выявлено, что на фоне лечения размеры опухоли значимо уменьшаются в течение первых 3 месяцев от начала лечения, вероятность уменьшения опухоли после 12 месяцев низкая. Терапия каберголином приводит к уменьшению размеров опухоли у 85 % больных с макропролактиномой. Наиболее высокая вероятность выраженного регресса опухоли отмечается у пациентов молодого возраста, с гигантским размером опухоли и уровнем пролактина более 300000 мЕд/л.

Prolactinomas account for approximately 40% of all pituitary<sup></sup>adenomas. Medical therapy with dopamine agonists<sup></sup>is highly effective in the majority of cases and represents<sup></sup>the mainstay of therapy. This data provides analysis of the efficacy of this treatment according to the size, localization, age, sex and prolactin level. We analyzed the results of treatment of 176 patients (109 male and 67 female with mean age 34 years) with mean follow-up period 6 month. Tumor size in 52% of patients was more than 36mm. Prolactin level was 2333 2218283 mU/l (median 37735). We found that the faster tumor shrinkage was during first 3 month of treatment and we didn"t saw any positive dynamic after 12 month. The better results could be taken in treatment of young patients (300000mU/l.
