Микробный спектр при гнойно-трофических нарушениях у пациентов с патологией сосудов

Abstract

Целью исследования является определение групп антимикробных препаратов для эмпирической антибиотикотерапии на основе анализа результатов микробиологических исследований раневого содержимого трофических язв. Были проанализированы 126 результатов бактериологических исследований, проведенных в Гомельском областном отделении хирургии сосудов за период 2006-2007 годы. Наиболее частое инфицирование трофических язв обусловлено Staphylococcus aureus (47%) и Pseudomonas aerugenosa (13,7%) или их ассоциациями с представителями нормальной микрофлоры. Часть больных была инфицирована только энтеробактериями (Enterobacter cloacae, Enterobacter agglomerans) и даже Streptococcus viridians. Оправданно применение фторхинолонов (ципрофлоксацин 59,8% чувствительных штаммов) карбопенемов (имипенем-52,1% чувствительных штаммов) или аминогликозидов (гентамицин-45,3% чувствительных штаммов) для эмпирической терапии до получения антибиотикограммы.

The study was aimed to determine range of microorganisms resided on skin lesions of vascular origin with purpose to order empirical antibiotics. 126 bacteriological investigations were performed at Gomel region vascular surgery department from 2006 to 2007. Most frequent contamination was by Staphylococcus aureus (47%) and Pseudomonas aerugenosa (13,7%) or by their associations with representatives of normal flora. Some patients were contaminated only by enterobacteria (Enterobacter cloacae, Enterobacter agglomerans) or Streptococcus viridians. Use of fluorquinolones (ciprofloxacin 59,8% of sensitive strains), carbopenemes (imipenem -52,1% of sensitive strains) or aminoglycoside (gentamicin -45,3% of sensitive strains) for empiric therapy until antibioticogramm is obtained.
