Целиакия у детей: решенные и нерешенные вопросы этиопатогенеза

Abstract

В последнее десятилетие благодаря интенсивному развитию новых иммунологических, морфологических и молекулярно-биологических методов достигнут существенный прогресс в представлении об этиопатогенезе целиакии. Расширилось и изменилось понятие о целиакии, которую в настоящее время определяют как иммуно-опосредованное мультисистемное заболевание, вызываемое глютеном и соответствующими проламинами у генетически предрасположенных индивидуумов, характеризующееся наличием разнообразных сочетаний глютен-зависимых клинических проявлений, специфических аутоантител, HLA DQ2 и DQ8 гаплотипов и энтеропатии. В соответствии с новыми представлениями рассматривается возможность изменения алгоритма диагностики целиакии, в особенности у лиц из групп риска.

In the recent decade, thanks to rapid development of new immunologic, morphologic and biologic methods we have achieved dramatic progress in understanding of celiac disease aetiopathogenesis. The whole image of the disease has been changed and enlarged now it's defined as an immunemediated multisystemic disease, that is caused by gluten and corresponding prolamines among genetically vulnerable individuals, that is characterized by a variety of gluten-dependant clinical features, specific antibodies, HLA DQ2 and DQ8 haplotypes and enteropathy. According to this novel approach, we regard possibilities of celiac disease diagnostic algorithm improvement, especially among patients from risk groups.
