Abstract

Целью нашей работы явилось описание результатов лечения пациентов с применением криобаллонной абляции (КБА) при пароксизмальной фибрилляцией предсердий (ПФП). За период с января 2014 г. по декабрь 2015 г. КБА выполнена 50 пациентам (26 мужчинам и 24 женщинам, средний возраст 57,16±2,48 лет). Отбирались пациенты без структурной патологии сердца, с неэффективной антиаритмической терапией. Давность аритмии от 1 года до 7 лет. Первичных пациентов с ПФП прооперировано 46 человек. КБА проводилась под местной анестезией в условиях рентгеноперационной с использованием криобаллонна 28 мм первой генерации и циркулярного диагностического электрода. Продолжительность операции: 117,4±23,1мин. Время флюороскопии: 35,25±8,84 мин. Оценка эффективности проводилась в раннем п/о периоде, через 3, 6, 12 месяцев на основании субъективной оценки состояния здоровья и качества жизни, 12 канальной ЭКГ, ЭКГ МТ. Из 46 пациентов 1 летальный исход, 1 гемоперикард потребовавший дренирования, 2 преходящих пареза диафрагмального нерва. Рецидив в госпитальные сроки возник у 7 пациентов. Через 6 месяцев отсутствие рецидивов ФП достигнуто у 36 человек (78.26%). Свобода от ФП с учетом приема антиаритмической терапии достигнута у 39 (84,78%) пациентов, без приема антиаритмической терапии у 32 (69.56%). В последующем 8 пациентам выполнялась РЧА УЛВ с положительным эффектом. Метод КБА для изоляции УЛВ при ПФП демонстрирует высокие результаты на протяжении всего времени наблюдения. Эффективность достигает 78.26% через 6 месяцев после вмешательства. К тому же сама методика является более простой в исполнении, и не требует от оператора большого опыта в выполнении катетерных вмешательств.

In this study we compared PVI using methods of cryoballoon ablation in patients with paroxysmal atrial fibrillation. Cryoballoon ablation (CRYO) was performed on 50 patients (24 women and 26 men. The average age of 57,16±2,48). The patients without structural heart disease and with ineffective antiarrhythmic therapy were selected for interventional treatment. The duration of the arrhythmia was from 1 year to 7 years. CRYO carried out by local anesthesia in the conditions with X-ray using cryoballoon 28 mm of the first generation and diagnostic circular electrode. The duration of surgery was 117.4±23,1min. fluoroscopy time was 35,25±8,84 min. Clinical efficacy was conducted in early postoperative period, after 3, 6, 12 months according to a subjective assessment of health status and quality of life, 12 lead ECG and ECG MT. In observed case analysis of the 46 patients, we received 1 death, 1 hemopericardium, which demanded drainage, 2 transient paresis of the phrenic nerve. Early recurrence in terms of hospital was detected in 7 patients. After 6 months of freedom from AF was achieved in 36 (78.26%). Freedom from AF based on receiving antiarrhythmic therapy was achieved in 39 (84.78%) patients without antiarrhythmic therapy in 32 (69.56%). In the subsequent 8 patients were underwent radiofrequency catheter ablation with a positive effect. CRYO for isolating PV demonstrates high results over the entire period of observation. The efficiency reaches 78.26% after 6 months after surgery. Thus, the method itself makes the performance easier, and does not require the operator a lot of experience in performing catheter procedures.
