Современные подходы к лечению предгипертонии у лиц молодого возраста

Abstract

Артериальная гипертония (АГ) остается одним из самых распространенных видов сердечно-сосудистой патологии. Число случаев заболевания, согласно эпидемиологическим данным, с возрастом прогрессивно растет. Накопленный опыт свидетельствует о том, что чаще всего наблюдается в молодом возрасте. По данным литературы частота АГ среди лиц молодого возраста составляет от 5 до 14%. Большое значение в возникновении первичной АГ придают наследственной предрасположенности, и ее относят к мультифакториальным заболеваниям. Для диагностики особенно важно учитывать наличие АГ у родителей главным образом, у матери с 60-80%. В реализации болезни не менее значимы и неблагоприятные средовые влияния: конфликтные стрессовые ситуации в семье, алкоголизм, активное и пассивное курение, гиподинамия. Повышенная масса тела и избыточное потребление поваренной соли представляют собой факторы риска АГ. Лечение пациентов АГ лекарственными препаратами рекомендуется начинать на основании уровня АД: на стадии «предгипертонии», но только при наличии сопутствующих факторов риска и признаками поражения органов-мишеней. При этом основное внимание уделяется снижению повышенного АД до целевого значения

Arterial hypertension (AH) is one of the most common types of cardiovascular events. According to epidemiological data the incidence of disease will progressively increase with age. Experience shows that it is mostly observed at young age. According to data from literature, frequency of AH among young people is from 5 to 14%. Inherited predisposition is of great importance at contradiction of primary AH, and it is referred to multifactorial diseases. It is very important to consider AH presence at parents, and mainly at mother with 60-80%. Adverse influence of environment is also significant at disease development: stressful situations of conflict in the family, alcoholism, active and passive smoking, hypodynamia. Increased body weight and excessive salt intake are risk factors for AH. It is recommended to start treatment of patients with AH by drugs on the basis of BP (blood pressure) level: at the stage of “prehypertension”, but only at presence of contributing risk factors and signs of target lesions. It is necessary to focus at reduction of elevated BP to target value.
