Abstract

Проведен анализ хирургического лечения 34 больных с острым мезентериальным тромбозом в бассейне верхней брыжеечной артерии в стадии некроза кишки и перитонита. Выделены 2 группы, из которых первую составили 24 (70,6%) пациента, ранее первично оперированных на этапе районных больниц. Во вторую вошли 10 (29,4) больных, первичное оперативное вмешательство которым выполнялось в специализированном центре. В группах применяли обструктивные резекции кишки с отсроченным анастомозированием и программированными санациями брюшной полости. Общая летальность составила 55,9%, в основном за счет больных первой группы. Установлена эффективность резецирующих операций без наложения анастомоза для последующего контроля динамики течения перитонита и оценки жизнеспособности кишечной стенки для наложения соустья, регистрируемой при помощи интраоперационного дуплексного сканирования в ходе программированных санаций брюшной полости.

The analysis of surgical treatment of 34 patients with acute mesenteric thrombosis in the basin of the upper mesenteric artery under bowel necrosis and peritonitis. Identified two groups, the first of which accounted for 24 (70,6%) patients, previously operated primarily at the stage of regional hospitals. The second included 10 (29,4) patients, primary surgery which was performed in a specialized center. The groups used obstructive bowel resection with delayed anastomosis and program bailouts abdomen. Overall mortality was 55,9%, mainly due to the patients of the first group. The efficiency of resecting operations without anastomosis to further control the flow dynamics of peritonitis and to evaluate the viability of the intestinal wall, as registered with intraoperative duplex scanning for imposing anastomosis during programmed sanitation of the abdominal cavity.
