Abstract

Динамика ремоделирования полостей сердца при формировании хронической постэмболической легочной гипертензии (ХПЭЛГ). Метод трансторакальной ЭхоКГ. Обследовано 55 пациентов с диагнозом ХПЭЛГ. С течением времени увеличивается конечный диастолический объем (КДО) правого желудочка (ПЖ) с 77,1 ± 28,1 до 125,5 ± 66,7 мл, р = 0,0037, ухудшается фракция выброса (ФВ) ПЖ (ФВ ПЖ уменьшается с 43,3 ± 10,5 до 39,5 ± 10,8 %, р = 0,12). Выявлена корреляция между уровнем давления в легочной артерии (ЛА PAP) и КДО ПЖ (r = 0,66 (р = 0,042)), а также между PAP и КДО ЛЖ (r = —0,68 (р = 0,023)). После первого эпизода острой тромбоэмболии легочной артерии и увеличения давности ХПЭЛГ повышается уровень легочной гипертензии, происходит дилатация кольца трикуспидального клапана, и увеличивается скорость трикуспидальной регургитации. Прогрессирует диастолическая дисфункция ЛЖ и ПЖ.

Dynamics of remodeling of heart cavities at forming chronic postembolic pulmonary hypertension (HPEPH). Method of transthoracic ECHO. 55 patients with HPEPH are surveyed. Eventually the final diastolic volume (FDV) of right ventricle (RV) is enlarged from 77,1 ± 28,1 to 125,5 ± 66,7 ml, р = 0,0037, RV emission fraction (EF) (FF of RV decreases from 43,3 ± 10,5 to 39,5 ± 10,8%, р = 0,12) worsens. Correlation between pressure level in pulmonary artery (PA PAP) and FDV of RV (r = 0,66 (р = 0,042)), and also between PAP and FDV of LV (r = —0,68 (р = 0,023)) is reveled. The level of pulmonary hypertension rises after the first episode of acute thromboembolism of pulmonary artery and augmentation of HPEPH prescription, dilatation of tricuspid valve ferrule, and rate of tricuspid regurgitation is enlarged. Diastolic dysfunction of LV and RV progresses.
