Abstract

У больных с нарушением функции щитовидной железы происходит развитие сиалоаденоза с нарушением секреторной функции. Ежегодно увеличивается количество больных с эндокринными сиалоаденозами, которые нуждаются в проведении совместных лечебно-диагностических мероприятий, как со стороны стоматолога, так и со стороны эндокринолога. Целью исследования было определение гистологических изменений и секреторной функции больших и малых слюнных желез при гипотиреозе. Оценку состояния слюнных желез проводили на основе клинических, лабораторных данных, и данных ультразвуковой сонографии. При исследовании было выявлено, что у больных с гипофункцией щитовидной железы уменьшается секреторная функция как больших, так и малых слюнных желез, что клинически проявляется развитием ксеростомии разной степени тяжести. По данным биопсии малых слюнных желез происходит атрофия секреторных ацинусов, развитие фиброза. Наличие полиморфной стромально-перидуктальной инфильтрации может свидетельствовать о вторичном развитии хронического воспаления. Данные сонографии и допплерографии больших слюнных желез указывают постепенное на увеличение размеров всех слюнной железы, возникновение гиперэхогенности тканей слюнных желез, ухудшение процессов микроциркуляции, а особенно выражены данные признаки в околоушных слюнных железах.

In patients with impaired thyroid function is the development of sialoadenitis with violation of secretory function. Every year an increasing number of patients with endocrine sialoadenosis, who need to conduct joint therapeutic and diagnostic measures, on the part of the dentist, and endocrinologist. The purpose of this study was to determine histological changes and secretory functions of the large and small salivary glands in hypothyroidism. Assessment of salivary glands was performed based on clinical, laboratory data, and data ultrawave sonography. We study revealed that in patients with hypofunction of the thyroid gland decreased secretory function of both large and small salivary glands that clinically is manifested with xerostomia of varying severity. According to the biopsy of minor salivary glands is the secretory acini atrophy, the development of fibrosis. The presence of polymorphic stromal-perioductally infiltration may indicate the development of secondary chronic inflammation. sonography and dopplerography of major salivary glands show a gradual size enlargement of all salivary glands, the occurrence of hyperechogenic tissues of the salivary glands, deterioration of microcirculation, and is especially pronounced these symptoms in the parotid salivary glands.
