Abstract

Objective. The study of the dynamics of the species composition of the microflora foci of surgical infection of soft tissues in the course of the disease and the nature of the intermicrobials interactions at changing of initiating microflora by nosocomial bacteria. Materials and methods. Bacteriological examination foci of surgical soft tissue infections in 550 patients with determination of the dynamics of the species composition of microorganisms was performed. Changing the growth characteristics of the bacteria, the presence of antagonism, changes antilysozyme activity and ability to form biofilms in the interaction of community-acquired microorganisms with nosocomial strains was determined. Results. The development of purulent necrotic complications in 91 patients in 75% of cases accompanied by changes in the species composition of bacterial pathogens. In these cases, changes in the species composition of the microflora of the foci of infection occurred decrease in the proportion of Gram-positive cocci and increase Gram-negative bacteria. The main direction of the dynamics of the species composition of the microflora of surgical infection foci of corresponded to a «species chain»: Streptococcus spp. → Staphylococcus spp. → Enterobacteriaceae spp. → Pseudomonas spp. The direction of the stimulating effect of the interaction of microorganisms with respect to growth characteristics and biological properties of bacteria coincided with the direction of the dynamics of the species composition and the direction of antagonistic and suppressive effects were largely reversed. Conclusion. The dynamics of the species composition of the microflora foci of of surgical infection of soft tissues in the course of the disease was to change community-acquired Gram-positive microorganisms hospital and was accompanied with inhibition of microbial community-acquired by nosocomial bacteria.

Цель. Изучение динамики видового состава микрофлоры очагов хирургической инфекции мягких тканей в течение заболевания и характера межбактериальных взаимоотношений при смене инициирующей микрофлоры госпитальными бактериями. Материалы и методы. Бактериологическое исследование очагов хирургической инфекции мягких тканей 550 больных проводили в динамике с определением видового состава микроорганизмов. Определяли изменение ростовых характеристик бактерий, наличие антагонизма, изменение антилизоцимной активности и способности формировать биопленки при взаимодействии внебольничных микроорганизмов с госпитальными штаммами. Результаты. Развитие гнойно-некротических осложнений у 91 больного в 75% случаев сопровождалось изменением видового состава возбудителей. В этих случаях происходило изменением видового состава микрофлоры очагов инфекции – уменьшение доли грамположительных кокков и увеличение – грамотрицательных бактерий. Основное направление динамики видового состава микрофлоры очагов хирургической инфекции соответствовало «универсальной цепочке»: Streptococcusspp. → Staphylococcus spp. → Enterobacteriaceaespp. → Pseudomonas spp. При этом направление стимулирующего эффекта взаимодействующих микроорганизмов в отношении ростовых характеристик и биологических свойств бактерий совпадало с направлением динамики видового состава, а направление антагонистических и подавляющих влияний было преимущественно противоположным. Заключение. Динамика видового состава микрофлоры очагов хирургической инфекции мягких тканей в течение заболевания заключалась в смене внебольничной грамположительной микрофлоры госпитальной и сопровождалась подавлением внебольничных микроорганизмов нозокомиальными бактериями.
