ОЦЕНКА ПЕРФУЗИИ МИОКАРДА МЕТОДОМ ОДНОФОТОННОЙ ЭМИССИОННОЙ КОМПЬЮТЕРНОЙ ТОМОГРАФИИ ПРИ ПРИЕМЕ β-АДРЕНОБЛОКАТОРОВ И БЛОКАТОРОВ КАЛЬЦИЕВЫХ КАНАЛОВ

Abstract

С помощью однофотонной эмиссионной компьютерной томографии сердца у больных ишемической болезнью сердца (ИБС) на фоне приема β-адреноблокаторов и блокаторов кальциевых каналов определялся коронарный кровоток, а также области наибольшей гипоперфузии. При динамическом наблюдении выявлено изменение и перераспределение перфузии миокарда. Определены закономерности изменения коронарного кровотока в зависимости от применяемой группы препаратов базисной терапии ИБС β-блокатор метопролол, блокатор кальциевых каналов амлодипин). Выделены показатели, обладающие прогностической значимостью в ожидаемой индивидуальной эффективности изученных препаратов. Указанный принцип позволяет наряду с традиционным нозологическим подходом к выбору терапии ИБС индивидуализировать терапевтическое воздействие в зависимости от характеристик микроциркуляции миокарда: распространенности и глубины нарушения перфузии.

Single-photon emission CT of the heart was used to observe coronary blood flow and reveal the maximum perfusion region in patients with coronary heart disease (CHD) treated with beta-adrenoblockers and calcium channel blockers. Dynamic observation showed changes and redistribution of myocardial perfusion. Specific features of coronary blood flow were demonstrated depending on the therapeutic modality (beta-blocker metoprolol, calcium channel blocker amlodipine). Parameters ofprognostic value were identified depending on the effectiveness of the agents used. This approach together with the traditional nosological one to the choice of CHD therapy permits to individualize the therapeutic influence depending on myocardial micriocirculatory features, such as the spread and depth of perfusion disturbances.
