Abstract

The carotid and femoral arterial intima-media thickness in an isolated sample of 207 men without clinical evidence of atherosclerosis (2414 observations) was investigated. Relationship with the main modified and not modified cardiovascular risk factors was studied. Revealed that risk factors such as age, systolic blood pressure and cholesterol, which are the main components of the total cardiovascular risk-stratification models, were significantly associated with the carotid and femoral arterial intima-media thickness. At the same time, 68% of its value depends on other factors (in this case – not counted). In order to evaluate the impact risk factors on the studied large sign – was performed a linear multifactor regression analysis. The dependent variable is the total number of plaques detected by ultrasound in all investigated vascular beds. As the variables influencing factors discussed in relation to which there was a significant relationship with the frequency of detection of plaques, namely: age, LDL cholesterol, HDL cholesterol and triglycerides. This result suggests that arterial intima-media thickness as a marker of subclinical atherosclerosis may have independent prognostic significance.

Исследована частота выявления атеросклеротических бляшек (АБ) в общей сонной и бедренной артериях в изолированной выборке 207 мужчин, не имеющих клинических признаков атеросклероза (2414 наблюдений). Изучена взаимосвязь с модифицируемыми и немодифицируемыми сердечно-сосудистыми факторами риска. Такие «большие» факторы риска, как возраст, систолическое артериальное давление и холестерин были значимо связаны с частотой выявления атеросклеротических бляшек в общей сонной и бедренной артериях. В то же время 68% значимости зависело от других факторов. Для того чтобы оценить влияние факторов риска на единственный изучаемый признак, был проведен линейный многофакторный регрессионный анализ. Зависимой переменной являлась суммарное количество бляшек, выявляемых методом УЗИ по всем изученным сосудистым бассейнам. В качестве влияющих факторов рассмотрены переменные, в отношении которых была выявлена достоверная связь с частотой обнаружения АБ, а именно: возраст, уровень ЛПНП, ЛПВП и триглицеридов. Полученный результат позволяет предполагать, что толщина показателя «интима-медиа» как маркер субклинического атеросклероза может иметь самостоятельное прогностическое значение.
