Дифференциальная диагностика паркинсонизма

Abstract

К экстрапирамидным расстройствам относят двигательные нарушения, обусловленные поражением базальных ядер и связанных с ними структур. Несмотря на достижения современной нейрофизиологии и нейрофармакологии, до сих пор до конца не ясен патогенез и зачастую неизвестна этиология многих заболеваний этого круга. В связи с этим в основе классификации экстрапирамидных расстройств лежит в первую очередь синдромологический подход. Прежде всего выделяют гиперкинетические и гипокинетические синдромы. Как понятно из названий терминов, гиперкинетические синдромы проявляются «избыточными» движениями гиперкинезами (тремор, дистония, атетоз, хорея, миоклонии и т. д.). Под гипокинетическими экстрапирамидными синдромами понимают, в первую очередь, синдром паркинсонизма, основным клиническим проявлением которого является ослабление функции движения. Целью данной работы является освещение вопроса дифференциальной диагностики в рамках синдрома паркинсонизма.

Extrapyramidal disorders include motor disorders caused by disturbance of the basal ganglia and related structures function. Despite of modern neurophysiology and neuropharmacology achievements, pathogenesis and etiology of extrapyramidal disorders still not fully understood. In this regard, their classification is based on syndrome approach. First of all, isolated hyperkinetic and hypokinetic syndromes are marked out. As it is clear from these terms, that hyperkinetic syndromes manifested with «excessive» movements hyperkinesis (tremor, dystonia, athetosis, chorea, myoclonus, etc.). Hypokinetic extrapyramidal syndrome includes Parkinson"s syndrome, the major clinical manifestation of which is weaken movement function. This article discusses the problem of differential diagnosis of parkinsonism.
