СРАВНИТЕЛЬНЫЙ АНАЛИЗ ЭФФЕКТИВНОСТИ СЕРОЛОГИЧЕСКИХ И ЭПИГЕНЕТИЧЕСКИХ МЕТОДОВ СКРИНИНГА РАКА ТОЛСТОЙ КИШКИ

Abstract

Цель: проведение сравнительного анализа серологических и эпигенетических методов скрининга рака толстой кишки для создания адекватных скрининговых и диагностических алгоритмов по выявлению колоректального рака. Материалы и методы: обследованы 204 больных, у которых рак толстой кишки был выявлен при проведении скрининговых мероприятий. Контрольную группу составили 56 здоровых людей. Скрининговые мероприятия объединяли следующие методики: иммунохроматографический тест кала на скрытую кровь, определение онкомаркеров в крови (раковоэмбрионального антигена, онкоантигенов СА 19-9, СА 242), метилированной ДНК гена септина 9 в крови. Методом ROC-анализа определяли диагностическую значимость тестов и их комплексирования.Результаты: диагностическая эффективность скрининга рака толстой кишки путем определения уровней в крови РЭА, СА 242 и СА 19-9 составляет, соответственно, 40,4%, 50,8% и 51,7%. Эпигенетический тест на наличие метилата ДНК гена SEPT9 в крови как скрининговая методика характеризуется более высокой диагностической чувствительностью 85,3%.Заключение: на начальном этапе скрининга рака толстой кишки оптимальным сочетанием методов является комбинация иммунохимического теста на скрытую кровь в кале при пороге 100 нг/мл, определения наличия метилата ДНК гена SEPT9 в крови.

Purpose: Comparative analysis serologic screening techniques of epigenetic and colon cancer for adequate screening and diagnostic algorithms for detection of colorectal cancer.Materials and methods: Surveyed 204 patients whose colon cancer was detected in screening activities. A control group consisted of 56 healthy people. Screening measures unite following techniques: Immunochromatographic fecal occult blood, determination of tumor markers in the blood (carcinoembryonic antigen, oncoantigens CA 19-9, CA 242), methylated DNA gene septins 9 in the blood. The ROC-analysis determined the significance of diagnostic tests and their aggregation.Results: Diagnostic efficacy of colon cancer screening by blood levels of CEA, CA 242 and CA 19-9, respectively, 40,4%, 50,8%,51,7%. Epigenetic testmetilat DNASEPT9of blood as screening method is characterized by a high diagnostic sensitivity-85,3%.Summary: At the initial stage of colon cancer screening method is a combination of the optimal combination of immunochemical test for the occult blood in feces in the threshold of 100 ng/ml, metilat DNA detection of SEPT9 gene in the blood.
