Опыт применения современных методов реперфузионной терапии у больных с ишемическим инсультом в неврологическом отделении № 2

Abstract

Эффективность и безопасность проведения традиционных хирургических вмешательств – каротидной эндартерэктомии и каротидной ангиопластики – в острейшем периоде ишемического инсульта до настоящего времени остается спорным вопросом. Современные рекомендации ограничивают реканализационную терапию использованием системного и селективного тромболизиса, а также тромбэмболэктомии. Однако активное применение традиционных хирургических вмешательств позволяет значительно улучшить эффективность оказания помощи пациентам с ишемическим инсультом в первые часы от момента начала заболевания. Приведен опыт Регионального сосудистого центра НИИ–Краевой клинической больницы № 1 по применению каротидной эндартерэктомии и каротидной ангиопластики в острейшем периоде ишемического инсульта, когда использование отличных от рядовых методов реканализационной терапии полностью оправдало себя и не сопровождалось осложнениями.

The efficacy and safety of such common surgical procedures as carotid endarterectomy and carotid angioplasty in acute period of ischemic stroke remain a controversial and unproven question. Current recommendations limit recanalization therapy using systemic and selective thrombolysis and thromboembolectomy. However, the active use of traditional surgical interventions can significantly improve the efficiency of treatment in patients with ischemic stroke within the first hours from the onset of the disease. The experience of the Regional Vascular Center in Regional Clinical Hospital № 1 in the use of carotid endarterectomy and carotid angioplasty in acute period of ischemic stroke, when the application is different from ordinary methods recanalization therapy was fully justified and was not accompanied by complications.
