Abstract

The Article is dedicated to the study of the most informative methods of early detection of acute destructive pancreatitis. The sensibility, specific character and efficiency of applied methods have been defined. A treatment and diagnosis algorithm based on staging and prevalence rate of the process has been developed and implemented into clinical practice. Assessment of efficiency of the algorithm has been carried out based on comparative analysis of patient groups matching by primary parameters during the follow-up period before and after practical application. Standardized approach to selection of treatment and diagnostic algorithm allowed to reduce the intensity of suppurative complications at serious pancratitis from 47,0% to 32,5%, postoperative lethality from 39,6% to 23,1% and general lethality from 33,3% to 15% (p

В статье изучены наиболее информативные методы ранней диагностики острого деструктивного панкреатита. Определена чувствительность, специфичность и эффективность применяемых методов. Разработан и внедрен в клиническую практику лечебно-диагностический алгоритм, основанный на стадийности и распространенности процесса. Проведена оценка эффективности разработанного алгоритма на основании сравнительного анализа сопоставимых по основным показателям групп больных в периоды наблюдения до применения и после его внедрения. Стандартизированный подход к выбору лечебно-диагностического алгоритма позволил снизить частоту гнойных осложнений при тяжелом панкреатите с 47,0% до 32,5%, послеоперационную летальность с 39,6% до 23,1% и общую летальность, соответственно, с 33,3% до 15% (р
