Структурно-функциональная рассогласованность различных отделов головного мозга при шизофрении: роль интегративной перцепции

Abstract

В статье на основе анализа большого количества публикаций, касающихся структурных изменений в различных отделах головного мозга, а также рассогласованности функционального взаимодействия между ними при шизофрении, обосновывается роль нарушения интегративной перцепции (которое, по-видимому, является первичным по отношению к исполнительской функции и лежит в ее основе) как существенного механизма в развитии заболевания. На феноменологическом уровне эти нарушения проявляются в виде расстройств комплексного восприятия окружающей обстановки, начиная от элементарных обстоятельств (например, снижение реакции на боль у больных шизофренией) и вплоть до сложного синтетического анализа социального контекста (эмоций окружающих людей, оценки их поведения и пр.). На основе данной гипотезы рассматриваются возможные подходы к психосоциальной терапии больных.

On the basis of analysis of literary sources concerning structural changes in different brain areas and inconsistent functional communication between them in schizophrenia, the author emphasizes the role of impaired integrative perception (which could be prior to executive function and could underlie it) as a significant mechanism in the development of disease. Phenome-nologically, these disturbances show as distorted comprehensive perception of environment: from minor circumstances (for instance, weaker reaction to pain in schizophrenic patients) to complex synthetic analysis of social context (emotions of others, evaluation of their behavior etc.). This hypothesis serves the basis for possible approaches towards psychosocial therapy.
