Комплексная диагностика акне с последующим дифференцированным выбором метода лечения в зависимости от типа и тяжести заболевания

Abstract

Объект исследования: основную группу клинико-лабораторных исследований составили 197 больных различными типами акне; группу сравнения 57 больных юношескими акне. Цель исследования: оптимизировать схему диагностики и лечения воспалительных акне с разработкой дифференцированного подхода к лечению акне с учетом клинико-морфологической формы, типа и тяжести заболевания. Методы исследования: определение типа, клинической формы и тяжести акне, определение гормональных показателей сыворотки крови, определение биохимических показателей сыворотки крови. Полученные результаты: разработана диагностическая карта комплексного обследования больного акне, выявлены критерии дифференциальной диагностики менструальных и гиперандрогенных акне; доказана более высокая по сравнению с системной антибиотикотерапией клинико-терапевтическая эффективность расширенного и дифференцированного применения КОК с антиандрогенным действием (Регулон и Новинет) у женщин с менструальными, гиперандрогенными и постменопаузальными акне средней и тяжелой степени тяжести; оптимизированной схемы монотерапии роаккутаном у больных с тяжевыми формами акне; цинктерала в составе комплексной терапии юношеских акне средней степени тяжести, на основе чего разработан алгоритм дифференцированного лечения акне в зависимости от типа и тяжести заболевания и сформулированы четкие показания для назначения КОК у женщин с акне.

197 patients with different types of acne and 57 patients with juvenile acne were studied. The aim of the study was optimization of acne diagnosis and inflammatory acne treatment by means of elaboration of differential approach to acne treatment in consideration of clinical-morphological form, type and severity of the disease. The following methods of the study were used determination of the type, clinical form and severity of acne, assessment of hormonal indices of blood serum, assessment of biochemical values of blood serum. Diagnostic card of acne patient complex examination was elaborated; criteria of differential diagnosis of menstrual and hyperandrogenic acne were worked out; higher in comparison with systemical antibioticotherapy clinical-therapeutical efficacy of broadened and differential application of combined oral contraceptives with antiandrogenic effect (Regulon and Novinet) in women with menstrual, hyperandrogenic and postmenopausal moderate and severe acne, of optimized roaccutan monotherapy in patients with severe forms of acne, of zincteral as a part of complex therapy in patients with juvenile moderate acne was proved. On the basis of the received data algorithm of differential acne treatment depending on the type and severity of the disease was elaborated and precise indications for administration of COCs in women with acne were formulated.
