Алгоритм оперативного лечения больных с переломами грудных и поясничных позвонков

Abstract

The analysis of the remote outcomes of expeditious treatment of patients with fractures of chest and lumbar vertebras is carried out. It is revealed that the number of mistakes and complications at use of the device of external fixing (suppuration, a malpozition, псевдартрозы, not unions), and also duration of stay in a hospital and frequency rate of treatment, significantly exceeds that at application of a transpedikulyarny clamp. Considering that surgical treatment was carried out at patients with comparable localizations and types of spinal fractures, the author offered schemes of algorithms of application of conservative treatment, transpedikulyarny pogruzhny devices, devices of external fixing. Besides, it is supposed that further application of hi-tech techniques of expeditious treatment will be carried out on the basis of the specialized neurosurgical centers opened at research institutes of traumatology and orthopedics or a neyrokhirugiya or at profile chairs of medical universities.

Проведен анализ отдаленных исходов оперативного лечения пациентов с переломами грудных и поясничных позвонков. Выявлено, что число ошибок и осложнений при применении аппарата внешней фиксации (нагноения, мальпозиции, псевдартрозы, несращения), а также длительность пребывания в стационаре и кратность лечения, существенно превышает таковое при применении транспедикулярного фиксатора. Учитывая, что хирургическое лечение проводилось у пациентов с сравнимыми локализациями и типами переломов позвоночника, автором предложены схемы алгоритмов применения консервативного лечения, транспедикулярных погружных устройств, аппаратов внешней фиксации. Кроме того, предполагается, что дальнейшее применение высокотехнологичных методик оперативного лечения будет проводиться на базе специализированных нейрохирургических центров, открытых в научно-исследовательских институтах травматологии и ортопедии или нейрохиругии или при профильных кафедрах медицинских университетов.
