Abstract

Изучена динамика общей активности трипсиновых протеиназ, общего калликреина и универсальных ингибиторов плазмы крови в период подготовки больных раком головки поджелудочной железы (РГПЖ, Т2-4N0M0) к панкреатодуоденальной резекции (ПДР). После каждого из двух этапов подготовительных процедур рост ингибиторной емкости крови наблюдался только в 1-е сутки при сохранении и активации протеолиза. Значимое снижение активности протеиназ в плазме крови формировалось к концу второй недели после аппаратного плазмафереза (АПФ). Непосредственно перед ПДР, на 30-е сутки после АПФ, протеиназо-ингибиторный дисбаланс восстанавливался. Осложнения после ПДР могут быть связаны с наблюдаемым дисбалансом в отношениях «фермент—фермент» и «фермент—ингибитор» системы трипсиноподобных протеиназ. Учитывая приведенные результаты, при составлении плана подготовительных процедур и сроков ПДР следует ориентироваться на срок не позднее 10–14-го дня после АПФ.

Dynamics of general activity oftrypsin proteinases, total kallikrein and universal blood plasma inhibitors during preparation of patients with cancer of the pancreatic head (CPH, Т2-4N0M0) for pancreaticoduodenal resection (PDR) was studied. After each of the two phases of preparatory procedures, an increase in inhibitory capacity of blood was observed on day 1 only with maintenance and activation of proteolysis. Significant decrease in proteinase activity in blood plasma developed by the end of the second week after automated plasmapheresis (APP). Proteinase-inhibitor imbalance was restored on day 30 after APP and immediately before PDR. Complications after PDR can be associated with enzyme-enzyme and enzyme-inhibitor imbalance in the system of trypsin-like proteinases. Taking into account the above results, in planning the preparatory procedures and PDR time one should consider period not later than day 10–14 after APP.
