Эндоскопическая диагностика и лечение полипов желудка у детей

Abstract

Целью исследования явилось улучшение результатов диагностики и лечения детей с полипами желудка. Работа основана на результатах обследования и лечения 316 детей с полипами желудка за период с 1995 по 2011 гг. Всем пациентам проведено обследование согласно существующим медико-экономическим стандартам. Обнаружено, что развитие полипов происходит на фоне воспалительных изменений слизистой оболочки желудка у 92,1% детей. Выявленная ассоциированность полипов желудка с хеликобактериозом в 53,9% случаев требует проведения эрадикационной терапии согласно существующим протоколам. Нами разработаны показания к оперативному лечению и динамическому наблюдению полипов желудка у детей. Эндоскопическая полипэктомия выполнена 62 детям.После проведения курсов медикаментозной терапии мы отметили уменьшение размеров полипа у 11% детей, а у 16% полип подвергся полной регрессии. Эндоскопически и гистологически обоснованный отбор пациентов для удаления полипов желудка, преди послеоперационная подготовка обеспечивают хорошие ближайшие и отдаленные результаты.

The aim of the study was to improve the results of diagnostics and treatment of children with polyps of the stomach. The study is based on the results of examination and treatment of 316 children with gastric polyps from 1995 to 2011. All patients were examined according to existing standards.It was revealed that the development of polyps occured against inflammatory changes of gastric mucosa in 92.1% of children. The determined association of Helicobacter pylori infection with gastric polyps in 53.9% requires eradication therapy according to existing protocols. We have developed the indications for surgical treatment and observation of gastric polyps in children. Endoscopic polypectomy was performed in 62 children.After a course of drug therapy, we noted a reduction in the size of polyps in 11% of children and 16% had a complete polyp regression. Endoscopically and histologically reasonable selection of patients for removal polyps of the stomach, pre-and postoperational preparation provide good short-and long-term results.
