О хирургической тактике лечения огнестрельных проникающих черепно-мозговых ранений в мирное время

Abstract

The 55 patients of Neurosurgical Department of Municipal Hospital №3 in Irkutsk-city have been examined as well as the material of Bureau of Legal Medicine 325 deceaseds during the period since 1990 to 1999 on bullet craniocerebral wounds. The comparative analysis of methods' of prophylaxis of purulent complications and outcomes of bullet craniocerebral wounds depending on a type of damaging factor, a place of a wound, volume and term of surgical intervention has been conducted. The investigation has shown that bullet craniocerebral wounds in peace time are severe damages of head in which the most number of patients die during pre-hospital stage or during the first hours at hospital. The best results of surgical treatment were in the group of patients who immediately after primary surgical treatment received continuous intracarotic ingusion of antibiotics through superficial temporal artery.

Изучен клинический материал нейрохирургического отделения ГКБ №3 г. Иркутска 55 больных и материал бюро судебно-медицинской экспертизы (СМЭ) 325 погибших за период с 1990 по 1999гг. по огнестрельным ранениям черепа и головного мозга. Проведен сравнительный анализ методов профилактики гнойных осложнений и исходов огнестрельных черепно-мозговых ранений в зависимости от вида повреждающего фактора, места ранения, объема и срока хирургического вмешательства. Исследование показало, что огнестрельные черепно-мозговые ранения мирного времени тяжелые повреждения головы, при которых подавляющее большинство пострадавших погибает на догоспитальном этапе или в первые часы после поступления в больницу. Наилучшие результаты оперативного лечения получены в группе больных, которым сразу после ПХО проводилась непрерывная интракаротидная инфузия антибиотиков через поверхностную височную артерию.
