Abstract

В результате проведенного комплексного исследования состояния шейки матки (оценка степени зрелости шейки матки по шкале Бишопа; комплексное ультразвуковое определение состояния родовых путей (длина шейки матки, толщина нижнего сегмента матки, диаметр внутреннего зева, задний угол шейки матки, расстояние между головкой плода и промежностью)) у беременных в сроке 41 неделя + 4 дня в процессе преиндукции родовой деятельности с анализом оксидативного статуса цервикальной зоны (аргинин, метаболиты NO (NO базальный и индуцированный, iNOS)) выявлено участие окислительного стресса в возникновении пускового механизма родовой деятельности. При этом в группе пациенток с пролонгированной беременностью выявлены достоверные отличия в цитохимических показателях цервикальной слизи (уменьшение содержания аргинина в 2,8 раза по сравнению с контрольной группой, при этом уровень базального (NO) и индуцированного метаболита оксида азота (iNOS) тесно коррелировали с уровнем аргинина и были выше более чем в 3 раза). Определена перспективность оптимизации тактики ведения доношенной и пролонгированной беременности с применением активаторов оксида азота (в частности L-аргинина).

As a result of a comprehensive study of the cervix (cervical maturity rating on a scale of Bishop; integrated ultrasound to determine the status of the birth canal (length of the cervix, the lower uterine segment thickness, internal diameter opening, rear corner of the cervix, the distance between the head of the fetus and perineum)) during pregnancy at 41 weeks + 4 days in the process of labour preinduktion with analysis of the oxidative status of cervical zone (arginine, metabolites of NO (NO basal and induced, iNOS)) were part of oxidative stress in causing the trigger of labor activity. The group of patients with prolonged pregnancies identified reliable differences in the cytochemical indicators of cervical mucus (decrease of arginine in 2,8 times compared with the control group, while NO bazal and induced nitric oxide metabolite (iNOS) were closely correlated with the level of Arginine, and were higher in more than 3 times).Prospect optimization tactics of defined term and prolonged pregnancy, using nitric oxide activators (L-arginine).
