Влияние бронходилататоров на вариабельность ритма сердца у больных хронической обструктивной болезнью легких

Abstract

Цель. Изучить влияние теофиллина пролонгированного действия (Теотард, KRKA) и комбинации салметерола и флутиказона (Серетид, GlaxoSmithKline) на временные и частотные показатели вариабельности ритма сердца (ВРС), частоту и количество аритмий у больных хронической обструктивной болезнью легких (ХОБЛ). Материал и методы. Проведено обследование 144-х госпитализированных больных ХОБЛ и 35-ти лиц контрольной группы. На 2-е и 14-е сутки пребывания в стационаре проводился анализ временных и частотных показателей ВРС и 24-часовое мониторирование ЭКГ Результаты. Терапия обоими препаратами сопровождалась повышением мощности в области высоких и низких частот спектра, увеличением отношения мощности низкои высокочастотных колебаний (LF/HF). Отмечено снижение частоты наджелудочковых и желудочковых аритмий. При этом под действием теофиллина происходило увеличение среднего суточного количества одиночных и парных наджелудочковых экстрасистол; после назначения комбинации салметерола и флутиказона динамика количества экстрасистол отсутствовала. Заключение. У больных ХОБЛ и нарушениями ритма сердца для бронхолитической терапии более целесообразно применение комбинации салметерола и флутиказона, чем теофиллина. РФК 2007; 4: 51-54

Aim. To study effect of long-acting theophylline (Theotard, KRKA) and combination of salmeterol and fluticasone (Seretide, GlaxoSmithKline) on heart rhythm variability (HRV) and number of arrhythmic episodes in patients with chronic obstructive pulmonary disease (COPD). Material and methods. 144 patients with COPD and 35 patients of control group were examined. The analysis of HRV and Holter monitoring were made f on 2th and 14th days. Results. Treatment with both drugs led to increase in power of lowand high frequencies and their ratio (LF/HF), decrease in rate of supraventricular and ventricular arrhythmias. Theophylline therapy raised in a number of single and pair supraventricular extrasystoles. Treatment with combination of salmeterol and fluticasone did not change a number of extrasystoles. Conclusion. Combination of salmeterol and fluticasone is more preferable as a broncholytic therapy for patients with COPD and heart rhythm disorders. Rational Pharmacother. Card. 2007; 4: 51-54
