Abstract

Цель. Установить факторы риска развития диабетической автономной кардиоваскулярной нейропатии (ДАКН), оптимизировать диагностику данного осложнения у детей и подростков с сахарным диабетом 1 типа (СД1). Материалы и методы. 101 пациент с СД1 5—17лет были обследованы с проведением оценки параметров вариабельности ритма сердца (ВРС), с проведением 4 кардиоваскулярных тестов. Результаты. Факторами риска развития ДАКН у детей и подростков с СД1 являются высокая вариабельность гликемии (более 9ммоль/л в сутки), систематические гипогликемические состояния и наличие поздних осложнений диабета. Заключение. Алгоритм диагностики ДАКН у детей и подростков с СД1, основанный на исследовании факторов риска развития данного осложнения и оценке прироста частоты сердечных сокращений в ортостатической пробе, обладает высокой чувствительностью и эффективностью.

Aim. To determine risk factors for diabetic autonomic cardiovascular neuropathy (DACN) and to optimize diagnostic procedures for children and adolescents with type 1 diabetes mellitus. Materials and methods. Heart rate variability (HRV) was assessed in 101 subjects with type 1 diabetes mellitus aged 5—17y.o. in 4 functional tests. Subjects with 3 positive tests out 4 were considered positive for DACN. Results. Increased glycemic variability (9+ mmol/l per day), regular hypoglycemic events and late diabetes complications were found to be DACN risk factors in children and adolescents. Conclusions. A diagnostic algorithm for DACN in children and adolescents with type 1 diabetes mellitus based on orthostatic HRV variability assessment shows high sensitivity and efficiency.
