Abstract

The article presents a retrospective analysis of the immediate results of prosthetic heart valves in 22 pediatric patients who had surgery in the cardiac surgery department of the National Research Center of Surgery. AN Syzganov over the past 5 years. Mortality in this group of patients was 4.6%, non-fatal postoperative complications occurred in 5 patients, accounting for 22.7%. Currently, prosthetic heart valves in patients childhood and adolescence is one of the pressing problems of cardiac surgery. Morphological and clinical aspects that distinguish these operations from those of adults, are manifestations of "restrictive" hemodynamics in small-diameter prosthetic implantation, as well as the development of the picture "break" the prosthesis and should be replaced at a larger size as the child grows [1,2]. Aortic valve prostheses small diameter is fraught with increasing frequency of postoperative mortality, preserving the large gradient between the left ventricle and the aorta, the lack of regression of left ventricular mass and a shortened term survival of patients in the late postoperative period [1,2,3,6,7,10]. According to the literature, a quarter of children who underwent surgery reconstruction of congenital abnormalities of the mitral valve, further required valve replacement [1,2,4,5,8]. The purpose of the study an analysis of the immediate results of prosthetic heart valves in children and adolescents. Material and methods. Retrospective analysis subjected to immediate results of surgical treatment of 22 patients who underwent valve replacement in combination with other interventions on the heart in the period 2009-2013. National Scientific Center of Surgery. AN Syzganov (clinical base KazMUNO). The age of patients ranged from 4 to 15 years, the average age of the patients was 12,3 ± 3,3 years. Among the patients studied was 15 (68.2%) boys and seven (31.8%) girls. The reasons for the development of valvular in 17 (77.3%) patients was congenital abnormality in 3 (13.6%) chronic rheumatic heart disease, and 1 (4.6%) miskomatoznaya degeneration of the mitral valve, and a further 1 (4.6%) patients secondary infective endocarditis.

В статье представлен ретроспективный анализ непосредственных результатов протезирования клапанов сердца у 22 пациентов детского возраста, прооперированных в кардиохирургическом отделении Национального научного центра хирургии им. А. Н. Сызганова за последние 5 лет. Летальность в данной группе пациентов составила 4,6%, нелетальные послеоперационные осложнения возникли у 5 пациентов, что составило 22,7%.
