Оценка методов амплификации нуклеиновых кислот, используемых в России для выявления Chlamydia trachomatis

Abstract

Целью данного исследования было оценить диагностические характеристики методов амплификации нуклеиновых кислот (МАНК), которые в настоящее время широко используются в России для выявления Chlamydia trachomatis. В исследование были включены 446 пациентов с симптомами урогенитальной инфекции (319 женщин и 127 мужчин). Пять тестов на основе полимеразной цепной реакции (ПЦР) и один тест, основанный на технологии NASBA (nucleic acid sequence-based amplification амплификация, основанная на последовательности нуклеиновых кислот), разработанных тремя российскими компаниями, оценивались при исследовании цервикальных и вагинальных образцов, полученных у женщин, и уретральных образцов и первой порции свободно выпущенной мочи у мужчин. В качестве референс, метода была использована ПЦР Cobas Amplicor (Roche Diagnostics, Mannheim, Germany). Распространенность хламидийной инфекции в исследуемой популяции составила 12,6 % Российские МАНК и Cobas Amplicor ПЦР показали высокий процент совпадения результатов (от 97,9 до 99,2 % для разных клинических образцов). Чувствительность, специфичность, прогностическая значимость положительных и отрицательных результатов российских тестов в различных образцах варьировала от 86,1 до 100 %, 99,1 до 100 %, 92,3 до 100 % и 98,2 до 100 %, соответственно.

The aim oi the present study was to evaluate the performance characteristics of nucleic acid amplification tests (NAATs) currently widely used in the detection of C. trachomatis in Russia. Totally, 446 consecutive symptomatic patients (319 females and 127 males) were included. Five polymerase chain reaction (PCR) assays and one real-time nucleic acid sequence-based amplification (NASBA) assay, developed by three Russian companies, were evaluated on cervical and vaginal samples from females and on urethral and first voided urine samples from males. As reference method, the Roche Cobas Amplicor PCR was used. The overall prevalence of C. trachomatis infection was 12.6 %. The Russian NAATs and the Cobas Amplicor PCR displayed a high level of concordance (97.9 % to 99.2 %). The sensitivities, specificity, positive predictive value and negative predictive value of the Russian tests in different specimens ranged from 86.1 % to 100 %, 99.1 % to 100 %, 92.3 % to 100 % and 98.2 % to 100 %, respectively.
