Резекция поджелудочной железы при операциях по поводу рака желудка

Abstract

The article shows the experience of radical operations with pancreas resection, performed in case of stomach cancer: 123 operation with distal resection and 18 – with proximal (including total pancreatectomy). The lethality rate in case of distal resections was 6, 5 %, in case of proximal – 11%. “The genuine” microscopic tumor invasion of pancreas body and tail led to 18,9±11.6% of survival rate, false ingrowth – to 40.6±11.1%. After pancreatoduodenal resections the best results were marked for the patients with tumor moving to duodenum, than in case of invasion into pancreas head. The special attention is given to surgery technics and to the description of the original ways of gland incision and anastomoses formation with a focus on individual approach.

В работе обобщен опыт радикальных операций с резекцией поджелудочной железы (ПЖ), выполненных по поводу рака желудка: 123 операции с дистальной резекцией и 18 – с проксимальной (включая тотальные панкреатэктомии). Летальность при дистальных резекциях составила 6.5%, при проксимальных – 11%. При «истинной» микроскопической опухолевой инвазии тела и хвоста ПЖ 5-летняя выживаемость составила 18.9±11.6%, при ложном врастании 40.6±11.1%. После панкреатодуоденальных резекций лучшие результаты получены у больных с переходом опухоли на 12-перстную кишку, чем при инвазии в головку ПЖ. Особое внимание уделено технике операций, описаны оригинальные способы пересечения железы и формирования анастомозов с акцентом на индивидуальный подход.
