Abstract

Цель исследования. Сравнительная оценка результатов применения эндоскопической резекции слизистой оболочки (ЭРСО) и метода эндоскопической диссекции подслизистого слоя (ЭДПС) у больных с поверхностными эпителиальными новообразованиями (ПЭН) желудка и двенадцатиперстной кишки (ДПК). Материал и методы. Дана подробная характеристика ٩٢ оперированных больных, сформулированы показания для ЭРСО и ЭДПС, описана методика вмешательств. Результаты. Выполнено 99 эндоскопических операций: ЭРСО - 79; ЭДПС - 20; дана сравнительная характеристика вмешательств по продолжительности, частоте резекции единым блоком и полноте резекции при раннем раке желудка. Произведена сравнительная оценка непосредственных, ближайших и отдаленных результатов оперативного лечения. Заключение. ЭДПС в отличие от ЭРСО обеспечивает бОльшую частоту удаления новообразований единым блоком и полноту резекции при раннем раке желудка, но характеризуется большей длительностью вмешательства при сопоставимом количестве осложнений. После выполнения ЭДПС в ближайший и отдаленный периоды рецидив ПЭН, в том числе раннего рака желудка, развивается реже, чем после ЭРСО.

Aim of study. Comparative assessment outcomes of endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) in patients with superficial epithelial lesions (SEL) of stomach and duodenum. Materials and methods. retrospective study includes 92 patients. The article contains detailed description of patients, superficial epithelial lesions, technique of EMR and ESD and indications for endoscopic procedure. Results. 99 endoscopic operations were performed: EMR 79, ESD 20. The mean operation time, en bloc resection rate, complete resection rate for early gastric cancer were assessed for EMR and ESD and compared in consideration of lesion’s size. Comparative evaluation of immediate, short-term and long-term outcomes of endoscopic resections was performed. Conclusion.ESD provides better en bloc and complete resection rates and minimal local recurrence compared with EMR, but ESD is more time-consuming technique with the same rate of complications.
