Ремоделирование сердца у детей после эндоваскулярной и хирургической коррекции дефекта межпредсердной перегородки

Abstract

Проведено исследование показателей ремоделирования сердца у детей с вторичными дефектами межпредсердной перегородки (ДМПП) после эндоваскулярной и хирургической коррекции порока. Анализ полученных данных показал, что крупные ДМПП приводят к нарушению диастолической функции левого желудочка (ЛЖ), восстановление которой происходит в первые сутки после эндоваскулярной коррекции дефектов и не менее чем через 6 мес. после хирургической коррекции. Максимальная редукция дилатации правых камер сердца после коррекции ДМПП регистрируется в ранний послеоперационный период и сохраняется в отдаленные сроки, независимо от способа коррекции порока. После пластики ДМПП наблюдается значительная депрессия механической активности левого предсердия с восстановлением параметров через год после операции.

The study of indices of heart remodeling was performed in children with secondary atrial septal defects (ASDs) after either endovascular or surgical repair of the defects. Data analysis showed that serious ASDs lead to diastolic left ventricular (LV) dysfunction. Recovery of LV function occurred during the first day following endovascular repair or after six months following surgical correction. Maximum reduction of dilatation of the right heart chambers after of ASD repair is recorded in the early postoperative period and keeps during follow-up, regardless of the method of defect repair. We found a significant depression in mechanical activity of left atrium after surgical repair with the recovery one year after the procedure.
