Abstract

La determinación del estado ácido base del Recién Nacido mediante los valores de gases arteriales y venosos en sangre de cordón luego del pinzamiento y corte, es un método rápido de evaluar objetivamente el estado del recién nacido; es importante conocer comportamiento de valores en gasometrías obtenidas mediante cordocentesis practicadas en pinzamientos habituales de cordón; donde se toma la decisión del pinzamiento con base en factores de riesgo y criterios clínicos y no por el tiempo transcurrido desde el nacimiento. Objetivos: Establecer si existen diferencias en comportamiento de gasometría pareada venosa y arterial, realizada en cordocentesis antes del primer minuto de nacimiento, en comparación con muestras pareadas venosas y arteriales de cordocentesis practicada en pinzamientos habituales de cordón Materiales y Métodos. Se realizó un estudio no experimental analítico observacional, en gasometrías pareadas venosas y arteriales, de cordocentesis pre-pinzamiento y pos-pinzamiento; procesadas en equipo GEM PREMIER 3000. Resultados: Se incluyeron 120 muestras correspondientes a 30 Recién Nacidos registrándose, valores de gasometrías pre-pinzamiento (menores un minuto), similares a los valores obtenidos en Plymouth (Inglaterra); y valores de gasometría venosas y arteriales pos-pinzamiento, que no presenta valores de riesgo a pesar del tiempo del pinzamiento, ofreciendo los beneficios de pinzamientos basados en criterios clínicos y no en tiempo predefinido. Abstract. The determination of the acid-base status of the fetus by gas values in arterial and venous cord blood after clamping and cutting, is a quick way to objectively assess thestatus of the newborn, it is important to know the behavior of blood gas values obtained by cordocentesis performed in protection cord clamping, where the decision is based clamping risk factors and clinical criteria rather than the time since the birth. Goals: Establish whether there are differences in behavior of venous and arterial blood gases paired, held in cordocentesis before the first minute of birth, compared with paired samples of arterial and venous cordocentesis practiced protected cord clamping. Tools (Equipment) and Methods: A study non experimental observational analytic in venous and arterial blood gases paired, of cordocentesis pre clamping and post-clamping; computer processed GEM Premier 3000. Results: A total of 120 samples from 30 newborns recorded, blood gas values pre-clamping (under a minute), similar to the values obtained in Plymouth (England), and values of venous and arterial blood gas post-clamping, which is not risk values despite clamping time, offering the benefits of clamping based on clinical criteria and not on predefined time.
