Abstract

Savoir définir, identifier et traiter une épilepsie idiopathique canine représente un défi clinique majeur, dans la mesure où 30 % des chiens atteints d'épilepsie idiopathique sembleraient ne pas répondre au traitement antiépileptique initié. Il est important de savoir distinguer la pharmacorésistance épileptique d'un phénomène de pseudo-résistance, à l'issue duquel un premier changement de stratégie thérapeutique peut être initié à tort. Pour ce faire, par le biais d'une étude bibliographique, cette thèse permet de proposer une définition de l'épilepsie idiopathique canine réfractaire au traitement à partir d'éléments retirés de la recherche vétérinaire et de la pratique clinique. Toute stratégie thérapeutique doit faire l'objet d'une réflexion autour du choix et du dosage moléculaires dans l'objectif de répondre à un premier critère de réponse qu'est la réduction d'au moins un facteur deux de la fréquence des crises, tout en considérant les effets secondaires et la qualité de vie du chien et de son propriétaire. L'illustration par trois cas cliniques met en évidence l'importance de l'analyse de plusieurs critères lors des suivis thérapeutiques, à savoir le dosage des concentrations sériques moléculaires, les effets secondaires, l'observance thérapeutique, qui, à l'issue d'un suivi partiellement ou non satisfaisant, justifient une réévaluation de la dose ou des molécules employées. Dès lors que l'administration successive d'au moins deux ou trois stratégies thérapeutiques sur un délai de cinq années dans le respect des critères définis ne permettent pas l'obtention d'une amélioration de la maladie épileptique, l'épilepsie idiopathique peut être qualifiée de réfractaire aux traitements entrepris.

Knowing how to define, identify and treat canine idiopathic epilepsy is a major clinical challenge as 30 % of dogs with idiopathic epilepsy don't appear to respond to antiepilectic treatment. It's important to distinguish epileptic drug resistance from pseudo-resistance, which may lead to an initial change in therapeutic strategy. To this end, definition of treatment-resistant canine idiopathic epilepsy, based on elements from veterinary research and clinical practice, is proposed through a literature review. Any therapeutic strategy must be carefully in terms of molecular choice and dosage, with the aim of achieving the primary response criterion of a reduction in seizure frequency by at least a factor of 2, taking into account side effects and quality of life of both dog and owner. The illustration of three clinical cases highlights the importance of analysing several criteria during therapeutic follow-up, i.e. determination of serum molecular concentrations, side effects and therapeutic compliance, which, at the end of a partial or unsatisfactory follow-up, justify a re-evaluation of the dosage or molecules used. Idiopathic epilepsy is considered to be refractory to treatment when at least two or three successive therapeutic strategies over a period of five years have failed to improve the epileptic condition, in accordance with the defined criteria.
