Abstract

ÖZET TAVŞANLARDA TALKIN %0.9'LUK SALIN SOLÜSYONU VE %1'LİK İODOPOVİDON İÇİNDEKİ ÇÖZELTİLERİNİN PLÖREDEZ ETKİSİNİN KARŞILAŞTIRILMASI Bu çalışma 01.01.2003-01.01.2004 tarihleri arasında Erciyes Üniversitesi Tıp Fakültesi Hakan Çetinsaya Deneysel Araştırma Merkezinde yapıldı. Amaç: Deneysel tavşan modellerinde talkın %0.9'luk şalin solüsyonu ve %1'lik iodopovidon içindeki çözeltilerinin plöredez etkisinin karşılaştırılması. Materyal ve Metod: Çalışma 30 adet Yeni Zelanda beyaz tavşanında yapıldı. Tavşanlar 2.0-2.5 kg ağırlığında olup hepsi erkekti. Tüm tavşanlara cerrahi yapılmadan önce 35mg/kg ketamin hidroklorid ve 5mg/kg xylazine hidroklorid intramüsküler ile hafif anastezi verildi. Anestezi sonrası tavşanların sağ hemitoraksı traş edilip bu bölge batikon ile temizlendi. Antisepsi sonrası ciltten 2 cm lik keşi vertebra ile sternum arasına yapıldı. Antisepsi sonrası ciltten 2 cm'lik keşi vertebra ile sternum arasına yapıldı ve 7. ve 8. interkostal boşluk kunt diseksiyon ile açılıp parietal plevraya kadar ilerlendi. 25 gouge' lık iğne plevral boşluğa konuldu. 2 mi 400 mg/kg asbestten serbest talk (pH=8.4) I. Grup tavşanlarda %0.9'luk serum fizyolojik içinde çözeltilerek, II. Gruptaki tavşanlarda ise %1'lik iodopovidone içinde çözülerek verildi. Cerrahi sonrası tüm tavşanlar bir ay sonra IV ölümcül dozda pentobarbital uygulanarak sakrifiye edildi. Bu tavşanların sağ hemitorakslan enblok çıkartıldı. Çıkartılan spesmenler%10'luknötral tamponlu formalin içerisinde 48 saat bekletildikten sonra patolojik olarak incelendi. Patolojik inceleme makroskopik ve mikroskopik olmak üzere iki şekilde yapıldı. Makroskopik olarak rvadhezyon skorlaması yapılırken mikroskopik olarak alveolar değişiklikler (kollaps, kanama, ödem, septal kalınlaşma), viseral değişiklikler (kalınlaşma, fibrozis) ve hücre değişiklikleri incelendi. Bulgular: Patolojik inceleme sonunda makroskopik adezyon skoru çözücü olarak %1 lik iodopovidone kullanılan grupta %0.9'luk serum fizyolojik kullanan gruba göre daha yüksek olarak tespit edildi (p< 0.05). Mikroskopik incelemede ise gerek alveol seviyesindeki değişiklikler, gerekse plevral seviyedeki değişiklikler iodopovidon kullanan grupta istatistiksel olarak daha anlamlı bulunmuştur (p<0.05). Hücre düzeyinde yapılan incelemede ise özellikle iodopovidone kullanılan grupta mezotel hücreler ile ilişkili nötrofil infiltrasyonu baskın olup bu gruptaki plöredez hızının akut dönemde daha fazla olduğu görülmüştür (p <0.05). Sonuç: Sonuç olarak talkın plöredez amaçlı kullanımında çözücü solüsyon olarak kullanılan iodopovidon güvenli, kolay bulunabilen, pahalı olmayan bir kimyasal ajan olup talkın iodopovidon ile süspansiyon haline getirilip kullanıldığında serum fizyolojik kullanımına göre plöredezde anlamlı derecede etkin bir kombinasyon sağlanmaktadır (p<0.05). ABSTRACT COMPARASION OF PLEURODETIC EFFECTS OF TALC WHICH IS SOLUTED BY %0.9, SALINE SOLUTION AND %1 IODOPOVIDONE IN EXPERIMENTAL RABBIT MODELS. This experimental study was done at Erciyes University Medical School, Hakan Çetinsaya Experimental and Clinical Research Center (DEKAM) between 01.01.2003 - 01.01.2004. Aim: Comparasion of pleuredetic effects of talc which is soluted by %0.9, saline solution and %1 iodopovidone in experimental rabbit models. Material and Method: Thirty New Zelland white male rabbits (weight range; 2.0 to 2,5 kg) were used for this study. All rabbits were anesthetized with ketamine hydrochloride, 35mg/kg,xylazine hydrochloride, 5mg/kg IM. The thorax was prepared for aseptic surgery by shaving the right chest wall and then sterilizing it with iodopovidone, Under aseptic conditions,2 cm skin incision was made between vertebra and sternum after that muscles in the seventh and eight intercostal spaces were bluntly disected to expose the parietal pleura.25- gouge needle was inserted into the pleural space. 2ml 400mg/kg sterilized asbestos free talc (ph=8,4) were given to first group (0.9% serum saline used as a solvent) and second group (10% iodopovidone used as a solvent) respectivly. All rabbits were killed one month after the administration of talc by an IV lethal dose injection of pentobarbital. The right hemitorax was removed enblock from the rabbits and the expanded specimens were submerged in a 10% nötral buffered formolin solution for at least 48 h. and send to pathology department. Pathologic evaluations were done as macroscopic and microscopic analysis. VIWhile macroscopic analysis were performed by adhesion scoring microscopic examination was done by some parameters as alveolar changes (collapse, hemorrhage, edema, septal thickinig), visseral fibrozis and thickining and cellular infiltration. Results: At the end of the histopathologic examination, macroscopic adhesion score was found higher in second group (iodopovidone used) in contrast to first group (0.9 % serum sale used) (p<0.05). In microscopic examination both alveolar and pleural changes were found statistically significant in second group (p<0.05). The cellular examination showed that neutrophilic infiltration which is interacted with mesothel cells was dominant especially in second group (iodopovidone used) (p<0.05). Accorrding to that result, in acute phase, plöredesis rate is high in second group. Conclusion: As a result, iodopovidone can be used plöredesis as a solvent of talc and that is very cheap, safe and easily obtainable chemical agent as talc as. if iodopovidone is used as a solvend of talc, the efSciancy of pleurodesis increases by talc- iodopovidone combination.
