Abstract

OBJECTIVE: The purpose of the study is to investigate quality of life (QoL) afterhospitalization and to detect whether early mobilization and cardiac rehabilitation (EMCR)programme applied during hospitalization has any effect on the perspectives of patients?quality of life in patients with Acute Myocardial ?nfarction (AMI). Besides, to test whethertheir activity levels have improved or not during the program with 6- minute walk test(6MWT) and to study the relationship between instruments of QoL and 6MWT.METHOD: Thirty patients diagnosed with AMI was performe to 6-Step EMCR program.Before rehabilitation, patients filled the Short Form-36 scale (SF-36) and MacNew HeartDisease Health-related Quality of Life (MacNew) Questionnaire. In the fourth step of theprogram, 6 MWT has been applied when they became mobilized. Before patients have beendischarged from hospital, QoL instruments and walking test have been repetitively applied.RESULTS: Significant decline has been observed in the physical and social parts of Macnew(p<0.05), there is no significant differences in global score. While significant decline has beenobserved in the physical role and social functioning parts of SF-36, significant increase hasbeen found in the vitality, emotional role, mental health, pain and general health parts. Astatistically significant increase has been found in 6 minute walk distance of patients(p<0.001). There exists no relationship between 6 MWT and parts of the questionnaires.CONCLUSION: A good medical care and application of EMCR ensure not to deteriorateQoL during hospitalization compared to before hospitalization on patients with AMI. 6 MWThas achieved to assess the efficiency of EMCR and a significant improvement has beenobserved. AMAÇ: Akut Miyokard ?nfarktüs'lü (AM?) hastalarda yatıs sonrasında yasam kaliteleriniölçmek ve yatıs sırasında uygulanan erken mobilizasyon ve kardiyak rehabilitasyonprogramının (EMKR) hastaların yasam kaliteleri ile ilgili görüslerine etki edip etmediğiniincelemektir. Ayrıca program sırasında aktivite düzeylerinin gelisip gelismediğini 6 dakikayürüme testi (6DYT) ile test ederek yasam kalitesi ölçütleri ve 6 DYT arasındaki iliskiyiarastırmaktır.GEREÇ VE YÖNTEM: AM? tanısıyla yatırılan 30 hasta 6 Adımlık EMKR programına alındı.Hastalar rehabilitasyondan önce Kısa Form-36 (KF-36) ölçeğini ve ?MacNew? Kalp HastalığıSağlıkla ?lgili Yasam Kalitesi Anketi'ni (?MacNew?) doldurdular. Programın IV. adımındamobilize olduklarında 6 DYT uygulandı. Hastalar taburcu olmadan önce yasam kalitesianketleri ve yürüme testi tekrarlandı.BULGULAR: ?MacNew? Anketinin fiziksel ve sosyal bölümünde anlamlı bir düsme saptandı(p<0.05), toplam skorda anlamlı bir değisiklik görülmedi. KF-36 fiziksel rol ve sosyalfonksiyon bölümlerinde anlamlı düsme; vitalite, emosyonel rol, mental sağlık, ağrı ve genelsağlık bölümlerinde anlamlı artıs saptandı (p<0.05). Hastaların 6 dakika yürümemesafelerinde istatistiksel olarak anlamlı artıs bulundu (p<0.001). 6 DYT ile anketlerinbölümleri arasında iliski görülmedi (p>0.05).SONUÇ: AM?'lü hastalarda; iyi bir medikal bakım ve EMKR uygulaması hastaların hastaneiçi yasam kalitelerinin hastane öncesine göre kötülesmemesini sağlamıstır. 6 DYT erkenmobilizasyon ve kardiyak rehabilitasyon programının etkinliğini değerlendirmede basarılıolmus ve anlamlı gelisme görülmüstür. Ancak yasam kalitesi anketlerindeki sorular direkfiziksel aktiviteyi yansıtmadığı için 6 DYT ile aralarında iliski bulunamamıstır.Anahtar Kelimeler: Akut Miyokard ?nfarktüsü, 6 Dakika Yürüme Testi, ?MacNew?
