İnsülin kullanan tip 2 diyabetik hastalarda insülin tedavi uyumu ve bunu etkileyen faktörlerin belirlenmesi

Abstract

Aim: In this study, it is aimed to investigate insulin treatment compliance and factors effecting insulin treatment compliance on type 2 diabetic patients.Method: 100 patients consulting Burdur Bucak Public Hospital İnternal Medicine polyclinic, 200 patients consulting Antalya Training and Research Hospital İnternal Medicine polyclinic and 200 patients consulting Akdeniz University Hospital Endocrine department were involved in this study and we made questionnaire to the patients.Findings: 65,2 percent of the patients were started to insulin treatment by an internal diseases specialist and it was determined that as the first insulin treatment, basal insulin treatment was prefered with 44 percent. During the study, it was determined that premix insulin treatment was the most common method with 38,2 percent. 58 percent of the participants took diabetes education and 91 percent were educated by diabetes nurse. It was determined that patients with hypoglycaemia were educated by doctors with higher rates and patients were educated by diabetes nurses (X2=5,13, p<0,05). In last month, the number of insulin applied by the patients who experience hypoglycaemia has a higher rate than the ones who doesn't. (t=3,23, p<0,05). The number of insulin doses applied by patients experiencing hypoglycaemia is higher than the ones who dosen't. (t=4,31, p<0,05). It was determined that doctors gave education in higher rates in groups who made dose skipping and diabetes nurses gave education in higher rates in groups who didn't make dose skipping (X2=21,05, p<0,05). The number of insulin dose applied by patients making dose skipping is higher than the ones who doesn't. (t=4,91, p<0,05). It was determined that the number of insulin applicaiton applied by patients making dose skipping is higher than the ones who doesn't skip dose(t=5,87, p<0,05).Results: It was found that the patients making more injection and using insulin in higher rates have a higher risk of hypoglycaemia. That being given education to the patients by diabetes nurse decreases the risk of hypoglycaemia and increases the insulin compliance. Amaç: Bu araştırmada, tip 2 diyabetli hastalarda; insülin tedavi uyumu ve bunu etkileyen faktörlerin belirlenmesi amaçlanmıştır.Yöntem: Burdur Bucak Devlet Hastanesi İç Hastalıkları polikliniklerine başvuran ve insülin tedavisi alan 100, Antalya Eğitim ve Araştırma Hastanesi İç Hastalıkları polikliniklerine başvuran ve insülin tedavisi alan 200, Akdeniz Üniversitesi Endokrinoloji Bölümü polikliniklerine başvuran ve insülin tedavisi alan 200 hasta çalışmaya alındı ve anket formu uygulandı.Bulgular: Hastaların %65,2'sine ilk insülin tedavisini İç Hastalıkları uzmanı tarafından başlanılmış olup ilk olarak %44 oranla bazal insülin tercih edildiği saptanmıştır. Çalışma esnasında en çok kullanılan %38,2 oranla premiks insülin olduğu belirlenmiştir. Katılımcıların %58'i diyabet eğitimi almış olup, %91'ine diyabet hemşiresi tarafından eğitim verilmiştir. Hipoglisemi yaşadıkları tespit edilen hastaların eğitimlerini daha yüksek oranlarda doktorlar verirken, yaşamayanların eğitimlerini diyabet hemşirelerinin verdikleri tespit edilmiştir (X2=5,13, p<0,05). Son bir ayda hipoglisemi geçiren hastaların uyguladıkları insülin sayısının hipoglisemi geçirmeyenlere göre daha yüksek düzeyde olduğu belirlenmiştir. (t=3,23, p<0,05). Son bir ayda hipoglisemi geçiren hastaların uyguladıkları günlük insülin dozu geçirmeyenlere göre daha yüksek bulunmuştur (t=4,31, p<0,05). Doz atlaması yapan gruplarda yapmayanlara göre daha yüksek oranda doktorların, doz atlaması yapmayan gruplarda ise daha yüksek oranda diyabet hemşirelerinin insülin eğitimi verdiği saptanmıştır (X2=21,05, p<0,05). Doz atlayan hastaların uyguladıkları insülin doz sayısının atlamayanlara göre daha yüksek düzeyde olduğu belirlenmiş (t=4,91, p<0,05), doz atlayan hastaların uyguladıkları insülin uygulama sayısının atlamayanlara göre daha yüksek düzeyde olduğu tespit edilmiştir (t=5,87, p<0,05).Sonuç: Daha fazla enjeksiyon yapan ve daha yüksek doz insülin kullananlarda hipoglisemi riski yüksek bulunmuştur. Hastalara diyabet hemşiresi tarafından eğitim verilmesi hipoglisemi riskini azaltmakta, insülin uyumunu arttırmaktadır. Anahtar kelimeler: Tip 2 diyabet, İnsülin tedavi uyumu, İnsülin Tedavisi
