Abstract

Выполнен анализ современных подходов к лечению пациенток с эндометриоидными кистами яичников (ЭКЯ) и бесплодием. Отмечено, что ЭКЯ является одной из основных клинических форм эндометриоза, часто сочетающихся с бесплодием; проанализированы аргументы «за» и «против» их хирургического лечения. Решение о выполнении оперативного вмешательства данной категории пациенток должно быть тщательно взвешенным и принято с учетом возможного снижения овариального резерва. Отмечена необходимость исследований, направленных на совершенствование выбора лечебной тактики при обнаружении ЭКЯ у пациенток, страдающих бесплодием, а также дальнейшего изучения влияния ЭКЯ на физиологические процессы в тканях яичника, определение роли ЭКЯ и хирургического лечения кист в снижении овариального резерва и вероятности самопроизвольного наступления беременности.

The analysis of modern approaches to treatment of patients with endometrioid cysts (EC) and infertility was performed. It was noted that EC is one of the main clinical forms of endometriosis, often associated with infertility; the arguments of pros and cons of their surgical treatment were analyzed. The decision to perform surgery in that category of patients should be thoroughly thought-out taking into account a possible decrease of ovarian reserve. Further studies are necessary in order to improve the management of patients with infertility and EC, to study the influence of EC on physiological processes in ovarian tissue, and to determine the impact of EC and its surgical treatment on ovarian reserve and spontaneous conception.
