Abstract

В настоящее время заболевания, вызванные вирусом папилломы человека (ВПЧ), - одни из самых распространенных болезней, передаваемых половым путем. Проблема менеджмента цервикальных интраэпителиальных неоплазий (CIN) актуализируется тенденцией к росту частоты данного заболевания (инфицированность ВПЧ в мире за последнее десятилетие увеличилась более чем в 10 раз), способностью некоторых типов ВПЧ инициировать злокачественные процессы, а также разработанными в последнее десятилетие мерами первичной профилактики, которые, по заключению Всемирной организации здравоохранения, должны полностью предотвратить цервикальный рак. Цель - определить частоту рецидивов ВПЧ-инфекции после комбинированного лечения заболеваний шейки матки. Материал и методы. Были обследованы 80 пациенток репродуктивного возраста с заболеваниями шейки матки с персистирующей в течение ≥3 лет ВПЧ-инфекцией, которые получили комплексное лечение (радиоволновая конизация шейки матки и 3 курса Инозин пранобекса). Результаты. Использование Инозин пранобекса в терапии доброкачественных заболеваний шейки матки, ассоциированных с персистирующей ВПЧ-инфекцией, в комбинации с радиоволновой деструкцией позволяет сократить сроки эпителизации после деструкции, повысить эффективность терапии, определяемую через 6 мес по данным кольпоскопии, цитологии и определению ДНК ВПЧ и максимально снизить вероятность рецидивов в последующие 3 года.

Currently, diseases caused by human papillomavirus (HPV) are one of the most common sexually transmitted diseases. The problem of cervical intraepithelial neoplasia (CIN) management is actualized by increasing tendency of this disease frequency (HPV infection has increased more than 10 times over the last decade in the world), ability of some HPV types to initiate malignant transformation, as well as by the measures of primary prevention developed in the last decade, which, according to the conclusion of the World Health Organization, should completely prevent cervical cancer. The aim of the study was to determine HPV infection relapse incidence after combination therapy of uterine cervical diseases. Material and methods. 80 patients of reproductive age with uterine cervical diseases and with ≥3 years of persisting HPV infection were examined and received combination therapy (radio-wave conization of the cervix and 3 courses of Inosine pranobex). Results. Administration of Inosine pranobex in therapy of benign uterine cervical diseases associated with persistent HPV infection, in combination with radio-wave destruction, allows to reduce the time of epithelization after destruction, to increase efficacy of therapy which was determined after 6 months according to data from colposcopy, cytology and the HPV DNA test, as well as minimize chance of recurrence in the next 3 years.
