Detekcija pinealnih cista transkranijskom sonografijom

Abstract

Cilj istraživanja: Transkranijska sonografija moždanog parenhima (TCS) do sada nije upotrebljavana u evaluaciji morfologije pinealne žlijezde. Cilj studije je pokazati mogućnost TCS e u registriranju signala pinealne žlijezde, njene cistične morfologije i pokazati korelaciju s MRI snimkama u pretpostavci da TCS može poslužiti kao alternativna metoda evaluacije pinealne žlijezde. Metode i pacijenti: U studiju je uključeno 14 pacijenata s novootkrivenom cistom pinealne žlijezde na MRI u i 30 kontrola. Snimanje TCS su proveli dva neovisna istraživača bez uvida u MRI rezultate na aparatu Aloka SSD-5500. Pinealna cista je označena kao svaka hipoehogena struktura unutar hiperehogene zone žljezdanog tkiva ili hipoehogena lezija sa ili bez septuma okružena tankom ehogenom linijom. Širine žlijezde i lezije su izmjerene u latero-lateralnoj i antero- posteriornoj projekciji te uspoređene s rezultatima MRI snimaka. Podaci su obradjeni ANOVA analizom. Rezultati: Veličina lezije je varirala od 3, 5 x5, 0 mm do 17 x 10 mm u dijametru. Rezultati nisu pokazali statistički značajnu razliku izmedju rezultata dva istraživača (p>0, 05) no nadjena je značajna razlika izmedju kontrolne skupine i skupine bolesnika s cistom (p<0, 05). Zaključak: Rezultati studije pokazuju mogućnost TCS e u detekciji pinealne regije uz dobru korelaciju s rezultatima MRI i značajnu podudarnost u rezultatima izmedju dva neovisna ispitivača. TCS se pokazala kao kompetitivna metoda MRI u evaluaciji pinealnih cista, a u slučajevima većih cističnih lezija omogućila je i bolju morfološku vizualizaciju.
