Características clínicas de la primera crisis epiléptica en niños atendidos en un hospital terciario de Lima, Perú

Abstract

Objetivo: Describir las características clínicas, los exámenes auxiliares y los tratamientos en niños atendidos por una primera crisis epiléptica en un hospital de nivel III-1 en Lima, Perú. Materiales y métodos: Estudio prospectivo, tipo serie de casos en pacientes mayores de un mes y menores de 14 años, atendidos por una primera crisis epiléptica entre diciembre de 2019 y noviembre de 2020. El diagnóstico de crisis epiléptica fue establecido por un neurólogo pediatra. Los datos se recolectaron mediante entrevistas a familiares y revisión de historias clínicas. Resultados: Se incluyeron 39 pacientes con una edad promedio de 52,6 meses. Las crisis generalizadas representaron el 82 %, predominando las tónico-clónicas generalizadas (61,5 %). Las principales etiologías fueron crisis febriles (35,9 %), infecciones agudas del sistema nervioso central (SNC) (18 %) y traumatismo craneoencefálico (12,8 %). Al ingreso, el 46,2 % recibió uno o dos medicamentos anticrisis epilépticas; al alta, el 33,3 % requirió medicación. Se realizaron electroencefalogramas en el 30,8 % de los pacientes, tomografías en el 33,3 % y resonancias magnéticas en el 20,5 %. Conclusiones: Las crisis tónico-clónicas generalizadas fueron las más frecuentes, teniendo como principales etiologías las crisis febriles, las infecciones agudas del SNC y el trauma craneoencefálico. Finalmente, un tercio de los pacientes requirió medicación al alta hospitalaria.
