Physical restraint in older people: an opinion from the Early Career Network of the International Psychogeriatric Association

Abstract

RÉSUMÉ L'Association psychogériatrique internationale (API) a exprimé des préoccupations importantes concernant l'utilisation de contraintes physiques chez les personnes âgées dans divers milieux de soins aux personnes âgées. À la suite d'une analyse approfondie de la littérature disponible, le Early Career Network (ECN) de l'API a formulé une collection de recommandations fondées sur des données probantes visant à guider l'utilisation des contentions physiques dans divers contextes de soins et groupes démographiques. Les contraintes physiques violent non seulement les droits de l'homme, mais soulèvent également d'importantes préoccupations en matière de sécurité qui ont un impact négatif sur le bien-être physique, psychologique, social et fonctionnel des personnes âgées. De plus, leur efficacité en milieu gériatrique reste insuffisante. Compte tenu de ces considérations, l'API et son ECN affirment fermement que l'utilisation de contraintes physiques ne doit être considérée que comme un dernier recours dans la prise en charge des personnes âgées.

RESUMEN La Asociación Internacional de Psicogeriatría (IPA, por sus siglas en inglés) ha expresado importantes preocupaciones sobre el uso de restricciones físicas en personas mayores en diversos entornos de atención de ancianos. Después de un extenso análisis de la literatura disponible, la Red de Carrera Temprana (ECN) de la IPA ha formulado una colección de recomendaciones basadas en evidencia destinadas a guiar el uso de restricciones físicas dentro de diversos contextos de atención y grupos demográficos. Las restricciones físicas no solo infringen los derechos humanos, sino que también plantean importantes problemas de seguridad que afectan negativamente el bienestar físico, psicológico, social y funcional de los adultos mayores. Además, su eficacia en entornos geriátricos sigue siendo inadecuada. Dadas estas consideraciones, el IPA y su ECN afirman firmemente que el uso de restricciones físicas solo debe considerarse como un recurso final en el cuidado de las personas mayores.

ABSTRACT The International Psychogeriatric Association (IPA) has expressed significant concerns over the use of physical restraints in older people across diverse aged care settings. Following an extensive analysis of the available literature, the IPA's Early Career Network (ECN) has formulated a collection of evidence-based recommendations aimed at guiding the use of physical restraints within various care contexts and demographic groups. Physical restraints not only infringe upon human rights but also raise significant safety concerns that adversely impact the physical, psychological, social, and functional well-being of older adults. Furthermore, their effectiveness in geriatric settings remains inadequate. Given these considerations, the IPA and its ECN firmly assert that the use of physical restraints should only be considered as a final recourse in the care of older people.

الملخص أعربت الجمعية الدولية للطب النفسي للمسنين عن مخاوف كبيرة بشأن استخدام القيود البدنية لدى كبار السن في مختلف أماكن رعاية المسنين. بعد تحليل مكثف للأدبيات المتاحة، صاغت شبكة الوظائف المبكرة (ECN) التابعة لوكالة تشجيع الاستثمار مجموعة من التوصيات القائمة على الأدلة والتي تهدف إلى توجيه استخدام القيود الجسدية في سياقات الرعاية المختلفة والمجموعات الديموغرافية. لا تنتهك القيود الجسدية حقوق الإنسان فحسب، بل تثير أيضًا مخاوف كبيرة تتعلق بالسلامة تؤثر سلبًا على الرفاه الجسدي والنفسي والاجتماعي والوظيفي لكبار السن. علاوة على ذلك، لا تزال فعاليتها في بيئات المسنين غير كافية. بالنظر إلى هذه الاعتبارات، تؤكد وكالة تشجيع الاستثمار وشبكة التواصل الاجتماعي الخاصة بها بشدة على أن استخدام القيود الجسدية يجب أن يعتبر فقط ملاذًا نهائيًا في رعاية كبار السن.
