Tödliche koronare Gasembolie nach Pulmonalvenenisolation – eine seltene Komplikation

Abstract

ZusammenfassungVorhofflimmern und -flattern stellen in Deutschland die häufigsten anhaltenden Herzrhythmusstörungen dar und werden zunehmend mittels ablativer Verfahren minimal-invasiv behandelt. Als seltene Komplikation eines solchen Verfahrens stellt die vorliegende Kasuistik den Fall einer tödlich verlaufenen Gasembolie der rechten Koronararterie im Rahmen einer Pulmonalvenenisolation mittels Kryoablation vor. Als Ursache für die Gasembolie ist eine akzidentelle Perforation der Pulmonalvene bzw. des linken Vorhofs mit Punktion eines Bronchialastes anzunehmen. Die Bedeutung postmortaler bildgebender Verfahren in Ergänzung zur Obduktion mit Blick auf die Feststellung der Todesursache wird unterstrichen.
