Abstract

Introduction : Les cardiopathies de l'enfant déterminent en Afrique un problème de santé publique. Cette étude avait pour objectifs de déterminer la prévalence, le devenir des cardiopathies congénitales et acquises et de formuler un plaidoyer pour une prise en charge efficace. Patients et méthode : Il s’agissait d’une étude prospective de type descriptif d’une durée de 2 ans allant du 15 mars 2022 au 17 avril 2024 au service de pédiatrie de l’hôpital Kamsar. Résultats : Un total de 1464 admissions, la cardiopathie de l’enfant au service représentait 22 cas soit 2% environs. La tranche d’âge de 1 à 59mois (63.63%) était la plus retrouvée, le sexe ratio M/F 1 ,2. Les motifs de consultations étaient dominés par la fièvre (81.82%), les difficultés respiratoires (77.27%), la toux (63. 64%). A l’examen physique, on notait une polypnée (81.82%), un battement des ailes du nez (72.73%), une cyanose (68.18%). Les cardiopathies congénitales représentaient 77.27% ; les formes acquises étaient présentes chez 22.73%. L’évolution clinique était favorable chez un cas de communication inter ventriculaire (CIV) type Roger ; quatre enfants (18.18%) étaient évacués pour réparation chirurgicale au Maroc par la compagnie des bauxites de guinée (CBG). Neuf cas (41%) sont en attente d’être évacués à l’étranger par une organisation non gouvernementale (ONG) étrangère. Cinq cas (22.72-%) de décès, deux (9.09%) enfants étaient perdus de vue. Conclusion : La cardiopathie au service de pédiatrie était fréquente chez les enfants plus jeunes. Le pronostic était mauvais dans les 2 formes sans interventions chirurgicales. Introduction: Childhood heart disease is a public health problem in Africa. The objectives of this study were to determine the prevalence and outcomes of congenital and acquired heart disease and to advocate for effective management. Patients and method: This was a prospective descriptive study lasting 2 years from March 15, 2022 to April 17, 2024 in the pediatric department of Kamsar hospital. Results: A total of 1464 admissions, pediatric heart disease in the department represented 22 cases or approximately 2%. The age group of 1 to 59 months (63.63%) was the most common, the sex ratio M/F 1.2. The reasons for consultation were dominated by fever (81.82%), breathing difficulties (77.27%), and cough (63.64%). On physical examination, polypnea (81.82%), nasal flaring (72.73%), cyanosis (68.18%) were noted. Congenital heart disease represented 77.27%; acquired forms were present in 22.73%. The clinical evolution was favorable in one case of Roger type interventricular communication (IVC); four children (18.18%) were evacuated for surgical repair in Morocco by the Guinea Bauxite Company (CBG). Nine cases (41%) are awaiting evacuation abroad by a foreign non-governmental organization (NGO). Five (22.72-%) cases of deaths, two (9.09%) children were lost to follow-up. Conclusion: Heart disease in the pediatric department was common in younger children. The prognosis was poor in both forms without surgical interventions.
