DIAGNOSTICS AND TREATMENT OF ADOLESCENTS AND YOUNG WOMEN WITH ABNORMAL UTERINE BLEEDING AND OVARIAN RETENTION CYSTS ON THE BACKGROUND OF EXTRAGENIAL PATHOLOGY

Abstract

The article presents the main features of clinical diagnostic and therapeutic approaches for the management of patients with abnormal uterine bleeding (AUB) in adolescents and young women with ovarian retention cysts (ORC) taking into account of extragenital pathology (EP).Material and methods. 125 patients with ovarian cysts and menstrual disorders (MD) were examined: 62 women 18-35 years old, - 1 group; 63 teenage girls 14-17 years old - 2nd group. The general clinical examination, laboratory, hardware methods, estimation of somatic and neuro-vegetative status were used.Results and their discussion. It has been established that in adolescent and in women of young reproductive age, ORC in 76.2% of patients occurs in the presence of EP and MD, 43.2% - on the background of AUB (in adolescents it is more common than in adults). Risk factors include: an unfavorable premorbid background, foci of chronic infection, acute and/or chronic stress, gynecological and obstetric pathology of mothers of patients. An ultrasound (including dopplerography) picture and a study of oncomarkers confirmed the benign nature of ORC, the presence of hyperplasia of the endometrium. Clinical manifestations of AUB were accompanied by pronounced hormonal shifts, hemostatic disorders on the background of posthemorrhagic anemia (in 64.8% of cases, more often in adolescents).The effectiveness of therapy depended on the speed of setting the correct diagnosis, an adequate assessment of the severity of the patient’s condition for choosing the hemostasis method and the treatment of post-hemorrhagic anemia. The first line of conservative therapy included the use of tranexamic acid, hormonal hemostasis.Conclusions. AUB is a severe violation of the menstrual function, at which in 43.2% of cases detected ORC. More often pathology occurs in adolescents. The course of the disease is characterized by the presence of severe hormonal, hemostatic disorders, anemization of patients. When conservative treatment of AUB in combination with ORC in adolescents and young women in the complex therapy are carried out, the using of tranexamic acid in tablet form or infusion, have high efficiency.
